明胶类血浆代用品对凝血和纤溶功能的影响

摘    要:目的观察明胶类血浆代用品高容血液稀释后对机体凝血和纤溶功能的影响。方法择期手术患者,随机分为3组:琥珀酰明胶组(gelofusine,GEL),聚明胶肽组(haemaccel,HAE),乳酸钠林格液组(lactatedRinger's solution)为对照组,每组15例。输液前、后测定激活的全血凝集时间(activatedclottingtime,ACT)、凝集速率、快速血小板功能、血小板功能、血红蛋白、红细胞比容、血小板计数、活化部分凝血活酶时间、凝血酶原时间、凝血酶时间、纤维蛋白原、组织纤溶酶原激活物,纤溶酶原激活物抑制物。结果输液后与对照组比较,HAE组ACT延长、血红蛋白降低具有统计学意义(P<0.05);GEL组血红蛋白、红细胞比容、血小板计数、凝集速率、纤维蛋白原降低,ACT延长有统计学意义(P<0.05)。结论HAE对ACT有显著影响,GEL引起ACT延长、凝集速率、纤维蛋白原降低均超过了血液稀释的限度,但仍在正常生理值范围内。

Abstract:ObjectiveTo evaluate the influences of blood coagulation and fibrinolysis after infusing different artifical plasma substitutes. MethodsPatients were randomly allocated to receiving intravenous infusion of succinylated gelatin gelofusine(GEL) group,polygeline haemaccel(HAE) group,or lactated Ringer's solution(control group), with 15 cases in each group. The venous blood samples were measured: including activated clotting time(ACT),clot rate, platelet function, hemoglobin,hematocrit,platelet count,activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time, fibrinogen,tissue plasminogen, plasminogen activator inhibitor(PAI). ResultsCompared with control group, HAE group showed lower hemoglobin,prolonged ACT obviously(P< 0.05); GEL group hemoglobin,hematocrit,platelet count,clot rate,fibrinogen were reduced,ACT was extended markedly(P< 0.05). ConclusionHAE has significant effect on ACT, GEL induced ACT increase, clot rate decrease, fibrinogen may result from of not only hemodilution but also their properties. but these changes are inside the normal range.
