| 摘 要： | 目的对腹腔镜腹股沟疝修补手术方式的选择、网片的大小及网片是否需要钉合等问题进行探讨。方法为212例腹股沟疝患者进行腹腔镜腹股沟疝修补术,其中腹腔内网片覆盖法(IPOM)18例,经腹腔腹膜前修补法(TAPP)20例,完全腹膜外疝修补法(TEP)174例,术后随访1～50个月。结果IPOM手术时间38～162min,术后出现粘连性肠梗阻症状2例,膀胱损伤1例,复发2例;TAPP手术时间52～136min,术后出现粘连性肠梗阻症状1例,腹股沟区疼痛1例,无复发;钉合网片的TEP手术时间36～187min,免钉合网片TEP手术时间30～62min,术后主要并发症为阴囊血清肿或血肿,共19例,腹股沟区疼痛4例,膀胱损伤1例,复发1例。结论腹腔镜腹股沟疝修补是安全可靠的手术,对腹股沟区解剖结构清楚的认识、熟练的操作和经验的总结是保证手术成功的关键。
