肠外营养对慢性阻塞性肺疾病急性加重期患者的影响

目的 研究肠外营养对慢性阻塞性肺疾病(COPD)急性加重期需人工通气且合并营养不良患者的影响,探讨呼吸功能不全患者合理使用肠外营养的方法。方法 选择近5年因COPD急性加重予呼吸机支持且合并营养不良患者40例,随机分为2组,每组20例,A组予肠外营养(PN)治疗,B组未行PN治疗,予普通膳食,比较两组患者各项营养指标及呼吸功能指标的差异。结果 A组平均住院时间(19.7±8.24)d,平均带机时间(7.57±4.34)d;B组平均住院时间(23.2±7.7)d,平均带机时间为(9.53±5.47)d,B组与A组比较均有显著差异(P<0.05)。A组的营养指标及呼吸功能指标均明显改善,与B组比较均有显著性差异(P<0.05)。结论 适当的营养支持有利于COPD患者的康复,是降低其死亡率的重要手段之一。

The effects of parenteral nutrition on patients with COPD with acute exacerbation

Objective To assess the effects of parenteral nutrition (PN)on patients with chronic obstructive pulmonary disease (COPD) in acute exacerbation, and appropriate application of PN support in these patients.Methods 40 critical patients with COPD were randomly divided into two groups, group A .treated by PN ;group B ,ordinary diet without PN. Outcome of two group patients including nutritional and pulmonary function indices were analyzed. Results The nutritional indices and pulmonary function indices of group A with PN support improved significantly, which are much better than those of group B. Conclusion COPD patients with acute exacerbation could benefit from suitable application of PN, which is essential to reduce mortality rate in patients with COPD.
