雾化吸入呋塞米对慢性阻塞性肺病急性发作患者肺通气功能的影响

摘    要:目的 探讨吸入呋塞米对慢性阻塞性肺病 (COPD)急性发作患者肺通气功能的影响。 方法 将 12 0例COPD急性发作患者随机分为 4组 ,每组各 30例 ,A组 (治疗I组 )吸入呋塞米 2 0mg(2ml,2 0mg/2m1) ,B组 (治疗Ⅱ组 )吸入呋塞米 4 0mg ,C组 (阳性对照组 )吸入 β2 受体激动剂 0 .5 %硫酸沙丁胺醇雾化溶液lml(5mg ,10 0mg/2 0m1) ,D组 (阴性对照组 )吸入生理盐水 5ml,观察 4组患者吸药前及吸药后 15分钟肺通气功能的变化。 结果 吸药后 15分钟B、C组用力肺活量 (FVC)、第 1秒用力呼气容积 (FEV1)、最大呼气流量 (PEF)与其正常预计值的百分比FVC %、FEV1%、PEF %均显著高于吸药前 ,FVC %增加显著 (P <0 .0 5 ) ,PEF %和FEV1%增加非常显著 (P <0 .0 1) ,并均高于A组吸药后。B组吸药后肺功能上述指标改善情况与C组比较差异无显著性意义 (P >0 .0 5 )。不良反应发生率 10 %。 结论 吸入呋塞米可改善慢性阻塞性肺病急性发作患者的肺通气功能 ,而且是安全的

Abstract:Objective To investigate the effect of inhal ed furosemide on lung function in patients with acute exacerbation of chronic obst ructive pulmonary disease (COPD). Methods One hundred and tw enty patients with acute exacerbation of COPD were randomly divided into A,B, C and D group,each group contained 30 cases. The patients in group A (therapy I ) inhaled furosemide 20mg (2ml,20mg/2ml), group B(therapy II )inhaled furosemide 40mg (4ml,20mg/2ml),group C(positive-control) inhaled 0.5% salbutamol sulfate respirator solution l ml (5mg, 100mg/20ml), and group D inhaled normal saline 5m l as placebo-control. The percent of the forced vital capacity (FVC), forced ex piratory volume in one second (FEV 1) and peak expiratory flow(PEF) to their no mal predicted value as FVC%, FEV 1% and PEF% were measured before therapy and a t 15 minutes after inhaling,respectively. Results In group B and group C,FVC%, FEV 1% and PEF% were significantly increased at 15 minutes after inhaling(FVC%,P<0.05;PEF% and FEV 1%, both P<0.01).However,these indices in group A showed no difference.There were no s tatistical significant changes between group B and group C(P>0.0 5).The adverse reactions rates of group B were 10%.Conclusion I nhaled furosemide can ameliorate the lung function, and it is safe to patiens wi th acute exacerbation of COPD.
