胃肠道间质瘤的影像诊断价值

摘    要:目的:探讨胃肠道间质瘤(GIST)的影像学特点及诊断价值。方法:回顾分析53例经手术和病理证实的GIST患者影像及临床资料。结果:病灶分布于食管1例,胃29例,小肠16例,大肠5例,胃肠道外2例。病理分型:良性GIST14例,交界性10例,恶性26例,未确定3例:临床表现为消化道出血占38%(20/53),腹部不适及腹痛占36%(19/53)。主要影像学表现为黏膜下富含血供的肿块,境界清晰光整,外生性为主,肿块内出血、坏死、囊性变明显,局部淋巴结无转移?结论:GIST患者的影像学表现有一定的特征性,对准确定位、良恶性的判断、指导临床治疗和估计预后有重要价值。

Abstract:Objective:To investigate the imaging features of gastrointestinal stromal tumors(GIST) and its diagnostic value. Methods:The clinical and imaging data of 53 patients with GIST proved by operation and pathology were analyzed. Results:Tumors located in esophagus in 1 cases,stomach in 29 cases,small bowel in 16 cases,colorectum in 5 cases and extra gastrointestinal sites in 2 cases.Immunohistochemical study diagnosed the GISTs as benign in 14 cases,intermediate in 10 cases and malignant in 26 cases.The common symptoms were gastrointestinal tract bleeding(20/53),vague abdominal discomfort and abdominal pain(19/53).The main imaging appearances included submucous hypervascular lesions with well-defined margins,predominating exophytic growth,obvious hemorrhage or necrosis and cystic changes within the mass and no metastases to local lymph nodes. Conclusion:The imaging of GIST manifests some characteristics.It is valuable in localizing precisely,discriminating benign or malignantin,guiding clinical therapy and estimating prognosis for GIST.
