摘    要:目的:用核磁共振(MRI)弥散张量成像(DTI)研究缺血性脑白质损害(ILA)患者常规MRIT2加权像上脑白质损伤区和一些形似正常脑白质(NAWM)区的变化及其与认知功能之间的联系。方法:对34例ILA患者和19例健康对照者进行常规MRI、DTI及神经心理学检查。结果:DTI扫描时,ILA组的脑白质损伤区及NAWM区的平均弥散系数(MD)值较对照组NAWM区的MD值升高,部分各向异性指数(FA)值降低;ILA组NAWM区的MD值变化和韦氏全智商及执行功能障碍相关,FA值的变化和简易智能状态检查量表(MMSE)衡量的总体认知损害相关。结论:DTI可显示ILA患者常规MRI上NAWM区的病理变化,其变化与认知障碍相关。

Abstract:Objective:The changes both within lesions and normal-appearing white matter load on T2-weighted MRI in patients with ischemic leukoaraiosis(ILA) was investigated by diffusion tensor MRI(DTI) and the relation between the changes of DTI parameters and cognitive dysfunction was assessed.Methods: Thirty-four patiens with ischemic leukoaraiosis and 19 healthy volunteers underwent DTI,conventional MRI,and neuropsychological assessment. Results: In ILA group,on DTI,diffusity was increased within lesions and in normal-appearing white matter compared with the control group,FA was reduced.The changes of MD in the NAWM correlates with full scale intelligence quotient(IQ) and executive dysfunction,the changes of FA in the NAWM correlates with global cognitive impairment assessed by the MMSE. Conclusion: DTI can reveal pathological changes in normal-appearing white matter load on T2-weighted MRI in patients with ischemic leukoaraiosis,which correlates with cognitive dysfunctions.
