MSCTA、3 D-TOF MRA在颅内动脉瘤诊断中的价值

摘    要:目的::探讨多层螺旋CT血管造影(MSCTA)、3D-TOF磁共振血管成像(MRA)在颅内动脉瘤诊断中的价值。方法:对经选择性数字减影血管造影(DSA)或动脉瘤夹闭术证实,并行MSCTA或3D-TOF MRA检查的15例颅内动脉瘤病例进行回顾性分析。结果:15例16个动脉瘤中,前交通动脉7个,后交通动脉4个,右侧颈内动脉2个,大脑中动脉3个;MSCTA及3D-TOF MRA检出其中15个,漏诊1个左侧大脑中动脉微小动脉瘤,敏感性为93.75%;MSCTA、3D-TOF MRA能较好显示动脉瘤形态、大小、位置、瘤颈及其与载瘤动脉、颅底骨结构的关系;13例MSCTA或3D-TOF MRA测量的瘤体大小与DSA及术中测量基本一致。结论:MSCTA、3D-TOF MRA能准确诊断颅内动脉瘤,可以作为对高危人群进行颅内动脉瘤早期筛查的常规方法。

Diagnostic value of MSCTA and 3 D-TOF MRA for intracranial aneurysm

Abstract:Objective:To investigate the value of multislice spiral computed tomography angiography( MSCTA) and three-dimensional time of fligt magnetic resornance angiography(3D-TOF MRA) in the diagnosis of intracranial aneurysm. Methods:Fifteen patients with intracranial aneurysm identified by selective digital subtraction angiography( DSA) or aneurysm clipping were examined using MSCTA or 3D-TOF MRA,and the clinical data were retrospectively analyzed. Results:Sixteen aneurysms in 15 patients included 7 aneurysms in ACoA,4 aneurysms in PCoA,2 aneurysms in right internal carotid artery and 3 aneurysms in MCA. Fifteen aneurysms were checked out by MSCTA and 3D-TOF MRA,one aneurysm in the left MCA was missed,the sensitivity of which was 93. 75%. MSCTA and 3D-TOF MRA could better display the morphology, size, location and neck of aneurysm, and the relationship between tumor and skull base. Tumor sizes of 13 cases detected by MSCTA or 3D-TOF MRA were coincidence to the results of DSA and operation. Tumor size of 1 case detected by MSCTA and 1 case detected by 3D-TOF MRA were not coincidence to the results of DSA. Tumor morphology of 12 cases detected by MSCTA and 3 cases detected by 3D-TOF MRA were similar to the results of DSA and operation. Conclusions:MSCTA and 3D-TOF MRA can accurately identified the intracranial aneurysm,which can used as a routine metheod in early screening of intracranial aneurysms in high risk population.
