摘    要:目的:探讨促红细胞生成素(EPO)对心肺复苏后大鼠肺脏的保护作用.方法:将大鼠随机分为假手术组、心脏复苏组(CPR组)及EPO治疗组(EPO组),ELISA法分析和比较各组大鼠肺脏组织丙二醛(MDA)、超氧化物歧化酶(SOD)及谷胱甘肽过氧化物酶(GSH-Px)的含量.结果:在自主循环恢复即刻,CPR组和EPO组的CaO2水平均低于假手术组(P<0.05),但随着恢复时间的延长逐渐恢复正常.心肺复苏3 h、6 h和12 h后,CPR组和EPO组肺脏MDA含量均明显高于假手术组(P<0.01).CPR组MDA含量12 h、6 h时间点显著高于3 h时间点(P<0.01).此外,在心肺复苏6 h、12 h后,EPO组MDA含量低于CPR组(P<0.05和P<0.01).心肺复苏3 h、6 h和12 h后,CPR组和EPO组肺脏SOD和GSH-Px均较假手术组降低(P<0.05~P<0.01).CPR组SOD和GSH-Px含量6 h时间点显著低于3 h时间点(P<0.01),12 h时间点则显著高于6 h时间点(P<0.01);而且,心肺复苏3 h、6 h和12 h后,EPO组GSH-Px和SOD含量均明显高于CPR组(P<0.01).EPO治疗前,大鼠肺脏MDA含量与SOD含量(P<0.05)和GSH-Px含量(P<0.01)均呈负相关关系.EPO治疗后,大鼠肺脏MDA与GSH-Px呈负相关关系(P<0.05),但与SOD无相关性(P>0.05).结论:EPO能降低心肺复苏后肺脏的氧化应激反应,减轻心肺复苏后的肺损伤程度,促进心肺复苏后肺脏功能的恢复.

Abstract:Objective:To evaluate the protective effects of erythropoietin( EPO) on the lung of rats after cardiopulmonary resuscitation ( CPR) . Methods:The rats were randomly divided into the sham group,CPR group and EPO group. The levels of MDA,SOD and GSH-Px in lung were detected using ELISA. Results:At the time of restoration of spontaneous circulation,the levels of CaO2 in CPR group and EPO group were lower than that in sham group(P<0. 05),and with the lengthening of restoration time,the level of which gradually returned to normal. After 3 h,6 h and 12 h of CPR,the levels of MDA in CPR group and EPO group were significantly higher than that in sham group(P<0. 01),the levels of MDA in CPR group after 6 h and 12 h of CPR were significantly higher than that after 3 h of CPR(P<0. 01),and the levels of MDA in EPO group after 6 h and 12 h of CPR were lower than that in CPR group(P<0. 05 and P<0. 01). Compared with the sham group,the levels of SOD and GSH-Px in CPR group and EPO group decreased after 3 h,6 h and 12 h of CPR(P<0. 05 to P<0. 01),the levels of SOD and GSH-Px in CPR group after 6 h of CPR were significantly lower than those after 3 h of CPR(P<0. 01),and the levels of SOD and GSH-Px in CPR group after 12 h of CPR were significantly higher than those after 6 h of CPR(P<0. 01). The levels of SOD and GSH-Px in EPO group after 3 h,6 h and 12 h of CPR were significantly lower than those in CPR group(P<0. 01). Before treatment with EPO,the content of MDA was negatively correlated with SOD(P<0. 05) and GSH-Px (P<0. 01) in rats. After treatment with EPO,the content of MDA was negatively correlated with GSH-Px(P<0. 05),but uncorrelated with SOD in rats(P>0. 05). Conclusions:EPO can reduce the oxidative stress and injury of lung,and promote the recovery of lung function after CPR.
