全胰管栓塞在胰十二指肠切除术中的应用

目的：防止胰十二指肠切除术并发胰空肠吻合口瘘。方法：在Ｗｈｉｐｐｌｅ手术中，残胰胰管采用ＴＨ胶栓塞缝合关闭，免除胰空肠吻合，结果：共实行残胰管栓塞术４２例，无１例术后胰瘘及手术死亡。术的一恢复顺利，均痊愈出院，全部病例随访１￣８年，无糖尿病发生。服用麦片能维持消化功能，生活质量良好。结论：残胰胰管栓塞术，可有铲防止Ｗｈｉｐｐｌｅ术后并发胰瘘，降低手术死亡率，是一种较为理想的术式。

Duodenopancreactomy in complete obstruction of pancreatic duct

?Objective:To study the prevention of fistula formation in pancreaticojejunal anastomose after duodenopancreactomy(Whipple's operation).Methods:In duodenopancreactomy,the stump of pancreatic duct was injected with TH glue.Embolism was formed and the pancreatic duct was closed.Consequently the pancreaticojejunostomy was avoided.Results:This method was employed in 42 cases of Whipple's operations.Not a single case of postoperative pancreatic fistula or operative death occurred,the rate of successful operation has been raised.Conclusions:The embolism formation of TH glue in pancreatic duct prevents effectively the complication of the pancreatic fistula in duodenopancreactomy and that reduces the operative mortality and time of hospitalization.
