基层医院开展流产后服务效果初探及分析

目的：调查我院开展人工流产后服务（ post abortion care ， PAC）的工作现状，为进一步改进基层医院PAC提供依据。方法：对我院开展PAC前（2011年1月∽12月）及开展PAC后（2013年1月∽12月）在本院行人工流产术的8273例女性进行基本情况登记（2013年流产女性增加问卷调查），并对结果进行统计分析。结果：经比较，2011年及2013年人流女性在年龄、文化程度、婚姻状况、重复流产情况及本次妊娠原因等情况方面，差异均有统计学意义（ p＜0．05）；非意愿妊娠原因中2011年及2013年人流女性均因避孕失败导致人流的比例最高，其次为未避孕（ p＞0．05）；开展PAC后，2013年人流女性的重复流产率明显低于2011年人流女性（ p＜0．05）。结论：基层医院人流患者特点为年龄偏小，未婚、未产率高，文化程度偏低，计划生育知识欠缺，有效避孕措施使用率低，医院内开展PAC能明显提高人流女性关于避孕和生殖健康知识的了解、掌握，使其更积极地采取有效避孕措施，从而显著降低再次流产率。因此应根据人流女性的人口学特点有针对性的加强 PAC，特别是提供贯穿流产全程的计划生育知识宣教及充分咨询。

A preliminary study and analysis on effect of post abortion care in grass-roots hospital

Objective：To investigate the work status of our hospital carrying out the post abortion service （ post abortion care , PAC） , and to provide the basis for Further improving PAC of grass -roots hospital.Methods：Before our hospital carrying out PAC （2011January to December） and after our hospital carrying out PAC （2013 January ~December）,we checked in the basic registration of 8273 female patients who Induced abortion in our hospital （ abortion women in 2013 increased questionnaire ） , and the results were statistically ana-lyzed.Results：By comparison, women who induced abortion in 2011 and 2013,their age, culture degree, marriage status, repeated abor-tion and the pregnancy reasons , the differences were statistically significant （ p〈0.05 ）; the unwanted pregnancy reasons of abortion women in 2011 and 2013 are mainly due to contraceptive failure ,followed by nocontraceptive （ p〉0.05）;after carrying out PAC , repeat-ed abortion was significantly lower （p＆lt;0.05）.Conclusion：The characteristics of abortion women in grassroots hospital are younger , un-married, not high yield, low educational level, be deficient in family planning knowledge , effective contraceptive use rate should be low , by carrying out PAC ,hospital could significantly improve the abortion women on the knowledge of contraception and reproductive health , encourage them to take effective contraceptive measures more actively , thus significantly reducing the rate of abortion .So we should ac-cording to demographic characteristics of abortion women to strengthen PAC , especially the full consultation about family planning knowl-edge throughout the whole process of abortion .
