多层螺旋CT及其后重建在急腹症诊断中的临床应用价值

目的：探讨多层螺旋CT（MSCT）及其后重建在急腹症诊断中的临床应用价值。方法：回顾性分析162例急腹症病例的多层螺旋CT表现,并与手术、病理或临床随访结果对照。结果：162例急腹症患者中急性炎症88例;消化道穿孔7例;肠梗阻15例;肠套叠3例;胆管结石8例;泌尿系结石17例;外伤10例;血管病变6例;脾梗塞3例;宫外孕2例;黄体血肿破裂1例;卵巢肿瘤蒂扭转2例。经手术和病理证实89例,临床随访证实73例。CT诊断符合率94.4%。结论：MSCT能快速显示急腹症的病变原因、部位、程度、病变范围,有无血管病变及脏器坏死,并可发现并发病及合并症的存在。多层螺旋CT是急腹症非常有价值的检查方法,部分病例可作为首选检查方法。

The Clinical Role of The Multi-slice Spiral CT (MSCT) and its Reconstruction Techniques in Diagnosis of Acute Abdomen

Objective：To explore the clinical role of the multi-slice spiral CT（MSCT） and its reconstruction techniques in diagnosis of acute abdomen.Methods： The performance of multi-slice spiral CT of 162 patients with acute abdomen were analyzed retrospectively,and the results of surgery,pathology or clinical follow-up as control.Results： MSCT scan show：in all 162 cases of acute abdomen patients,88 cases of acute inflammation;7 cases of gastrointestinal perforation;15 cases of intestinal obstruction;3 cases of intussusception;8 cases of common bile duct stones;17 cases of urinary calculi;10 cases of trauma;6 cases of vascular disease;3 cases of splenic infarction;2 cases of ectopic pregnancy;1 case of ruptured corpus luteum hematoma;2 cases of torsion of ovarian tumor.89 cases were confirmed by the results of operation and pathology,73 cases were confirmed by clinical follow-up.The accuracy of MSCT is 94.4%.Conclusion： MSCT can quickly show that the etiology,location,extent,lesion area of acute abdomen,with or without vascular disease and organ necrosis,and the existence of complicating diseases and complications.Multi-slice spiral CT is a valuable technique in the evaluation of acute abdomen,In some cases it can be used as the preferred method.
