胸骨正中劈开入路治疗缩窄性心包炎临床分析

目的：探讨手术治疗缩窄性心包炎的最佳手术径路。方法：对１９７８年来收治的缩窄性心包炎１０６例病人行手术治疗，其中经左胸前外侧切口入路６６例，经胸骨正中劈开入路４０例，对１０６例病的手术及术后疗效，并发症进行回顾性分析。结果：经左胸前外侧切口手术治愈率为３４．９％（２３／６６），胸腔及肺部感染发生率高，死亡率为６．１％（４／６６），经胸骨正中劈开入路手术治愈率为８７％（３５／４０），无胸腔感染并发症

The Clinical Study on the Operation for Constrictive Pericarditis by Mid-sternal Incision

Objectives:To explore the best operative incision for the treatment of constrictive pericarditis.Methods:Results of 106 cases suffering from constrictive pericarditis were summarized,among them 66 cases underwent operations by the incision of left anterior lateral chest,40 cases had operations by the midsternal incisions.Results:The curative and mortality rates of the patients with the incision of left anterior lateral chest were 34.9%(23/66) and 6.1%(4/66) respectively,which of the others were 87.0%(35/40)( p <0.01) and 0( p <0.01).The postoperative complications developed in most of the patients with the incision of left anterior laleral chest.no complication occured in the cases with the midsternal incision.Conclusions :The midsternal incision was the best operative way for the operation of constrictive pericarditis with full exposure and less postopeative complications.
