血浆内皮素水平对癫痫的诊断和鉴别诊断价值

摘    要:目的 探讨血浆内皮素 (ET)水平对癫痫的诊断和鉴别诊断价值。方法 对 76例癫痫样发作患者在发作间期进行单光子发射计算体层摄影术 (SPECT)脑血流灌注显像、EEG检查、X- CT或 MRI扫描 ,与 2 5例正常对照用放射免疫法同时测定其血浆中 ET质量浓度。结果  76例癫痫样发作患者中 4 9例特发性癫痫患者血浆 ET质量浓度明显高于正常对照组 (P<0 .0 1) ,2 1例症状性癫痫 ET质量浓度与正常对照组无显著性差异 (P>0 .0 5 ) ,6例非癫痫性疾病患者血浆 ET质量浓度明显高于正常对照组 (P<0 .0 5 )。结论 血浆 ET质量浓度水平对癫痫样发作患者具有鉴别诊断价值 ,并能为特发性癫痫的定性诊断提供帮助。

Abstract:Objective To observe the level of serum endothelin (ET) of interictal epilepsy patients with regional cerebral blood flow (rCBF) decreased foci in SPECT brain perfusion imaging and to investigate its value for diagnosis and differential diagnosis of epilepsy. Methods Seventy-six interictal epilepsy patients were enrolled and SPECT brain perfusion imaging, EEG, X-CT or MRI were performed,with 25 healthy volunteers as controls whose serum ET levels were measured by radioimmunoassay. Results The serum ET levels of 49 idiopathic epilepsy patients were significantly higher (P<0.01) than that of healthy controls. The ET levles of 21 symptomatic epilepsy patients were not significantly different from that of the healthy controls (P>0.05). The ET levles of 6 non-epilepsy patients were significantly higher than that of the healthy controls (P<0.05). Conclusion The level of serum ET is of value to the differential diagnosis for interictal epilepsy cases, and can contribute to the qualitative diagnosis of idiopathic epilepsy.
