糖尿病肾病早期实验室检测指标与病程关系的探讨

目的:探讨糖尿病肾病早期实验室监测指标及其与糖尿病病程的关系。方法对306例糖尿病不同病程患者及100例健康对照者尿蛋白定性和尿微量白蛋白(mAlb)、-微球蛋白(-m)和免疫球蛋白G(IgG)定量检测结果进行统计分析。方法:对306例糖尿病不同病程患者及100例健康对照者尿蛋白定性和尿微量白蛋白(mAlb)、-微球蛋白(-m)和免疫球蛋白G(IgG)定量检测结果进行统计分析。结果:糖尿病(DM)组尿蛋白定性及尿微量白蛋白(mAlb)、-微球蛋白(-m)、和免疫球蛋白G(IgG)定量检测异常结果均明显高于健康对照组,其差异有统计学意义;糖尿病组中,尿蛋白定性阳性率22.5%;尿mAlb、-m、IgG阳性率分别为54.9%、63.7%、和22.7%;尿mAlb和尿-m联合检测总阳性率为75.5%,且糖尿病患者随病程进展尿mAlb和尿-m水平不断升高,尿蛋白定性、尿mAlb和尿-m阳性检出率不断增高。结论:糖尿病患者尿蛋白定性阴性不能排除早期肾损伤,尿mAlb和尿-m检测是糖尿病肾病早期诊断和病程监测的灵敏指标。对于糖尿病患者,定期进行尿mALB和尿-m指标的单项或联合检测,可以早期发现患者肾功能改变,有利于防止和延缓的糖尿病肾病(DN)发生。

Study the relation of the Laboratory Parameter and the course of disease in EarIy Diagnosis Of Diabetic Nephropathy

Objective To Study the relation of the Laboratory Parameter and the course of disease in EarIy Diagnosis Of Diabetic Nephropathy.Methods Detecting blood glucose of hungry state,qualitative urinary protein,urine mAlb and urine-m of 306 cases of diabete patients in different progress of the disease and 100 cases of control group.Results Positive rates of urine protein,urine mAlb and urine-m are 22.5%,54.9%、63.7% and 22.7%,respeciively.Total positive rate of union detection of urine mAlb and urine-m is 75.5%.As the progressing of disease,urine mALB,urine-m positive rate,the union detection of qualitative urine protein,urine mAlb and urine-m become higher continuously.Conclusion The negative result of qualitative urine protein can not exclude early nephropathy of diabetes.Union detection of urine mAlb and urine-m is of great importance for early discovery diabetes nephropathy.
