CT与MRI对婴儿型肝脏血管内皮瘤的诊断效果观察

目的:探讨CT、MRI对婴儿肝脏血管内皮瘤的诊断价值。方法:回顾性分析19例婴儿肝脏血管内皮瘤的CT、MRI表现,15例行CT检查,6例行MRI检查(其中2例同时行CT和MRI检查),并结合文献加以讨论。结果:19例中单发者9例,多发者3例,弥漫性7例。15例CT平扫为单发或多发肿块呈低密度,边界清楚,增强后小病灶明显均匀强化,大病灶从周边向中心强化,延时后均匀强化,11例见粗大供血动脉进入肿块内。6例MRI平扫肿块T1WI呈低或等信号,T2WI呈高信号,部分内见流空血管。结论:婴儿肝脏血管内皮瘤增强后有特征性的CT、MRI表现,多可与肝脏其他实性肿块鉴别作出正确诊断。

The CT and MRI diagnosis of Infantile Hepatic Hemangioendothelioma

Objective To approach the diagnostic value of CT and MRI in the diagnosis of infantile hepatic hemangioendothelioma.Methods We retrospectively reviewed the imaging features in 19 cases and made discussion according to references.Fifteen patients were performed CT non-enhanced scanning and enhancement.MR imaging were obtained in six patients,both CT and MRI were performed in two case.Results In the 19 patients,9 had single mass,3 had multiple mass,7 had diffuse mass.CT scan show as a well-defined single or multiple mass with lower attenuation than that of the normal liver parenchyma in 15.The mass enhanced gradually from periphery to the central area in the macro-focus,significant homogeneous enhancement within tumor mass in the micro-focus.A dilated and elongated feeding artery may be seen in the mass in 11.At unenhanced MR imaging,the lesions have low or equal signal intensity on T1-weighted images and high signal intensity on T2-weighted images in 6.Flowing void vessels may be present on T2-weighted images in some cases.Conclusion The contrast-enhanced CT and MRI findings of infantile hepatic hemangioendothelioma have some characteristic features and differential diagnosis form other solid hepatic tumors can be made possible.
