四孔法腹腔镜胆囊切除术与传统开腹胆囊切除术的临床疗效比较

目的：对比分析传统开腹胆囊切除术与四孔法腹腔镜胆囊切除术（LC）的手术安全性及疗效，探析治疗胆囊疾病的最佳术式。方法：分析2013年3月～2015年11月在我院接受胆囊切除术治疗的80例胆囊炎患者的临床资料。根据手术方式的不同将入选者分成观察组（四孔法LC，40例）和对照组（传统开腹胆囊切除术，40例）两组。比较两组患者的基线资料、手术治疗效果及并发症发生率。结果：两组患者的基线资料无统计学意义；观察组患者的手术时间、切口长度、术中出血量、住院时间均明显低于对照组，且术后疤痕满意度评分高于对照组；观察组患者的并发症（包括切口出血与感染、胆漏、放射痛、胆管损伤）发生率明显低于对照组。结论：与传统开腹胆囊切除术相比，四孔法LC治疗胆囊疾病患者的手术时间更少、术中出血量更少、并发症发生率更低，给患者带来的痛苦及创伤更小，值得推广。

The comparison of the clinical effects of four-port LC and traditional open cholecystectomy

Objective Our retrospective study was aimed to comparatively analyze the operation safety and curative effect of traditional open cholecystectomy and four-port laparoscopic cholecystectomy(LC),and analyze the best operation to treat the gallbladder disease. Methods Clinical data of 80 patients with cholecystitisreceived treatment at our hospital from March,2013 to November,2015 was retrospectively analyzed. Patients included were divided into two groups according to the treatment, observe group(four-port LC,40 cases)and control group(traditional open cholecystectomy,40 cases). The general information, surgical treatment effect and complication rate of patients between two groups were compared. Results The general informa-tion of patients in two groups had no statistical difference. The operation time,incision length,intraoperative blood loss,length of stay of patients in observe group were obviously lower than those in control group. The complication rate(includingincision bleeding and infection,bile leakage,radiation pain,bile duct injury)of patients in observe group was obviously lower than that in control group. Conclusion Comparing to the traditional open cholecystectomy,the four-port LC has short operation time,less blood loss,low complication rate,small pain,small wound. The four-port LC is worth promoting.
