MSCTA 在颅内动脉瘤诊断中的应用价值分析

目的：MSCTA（多层螺旋 CT 血管造影）在颅内动脉瘤诊断中的应用价值分析。方法：选取在本院就诊的动脉瘤蛛网膜下腔出血的患者37例，术前均行 MSCTA 和 DSA（数字减影血管造影）检查。比较 MSCTA 和 DSA诊断颅内动脉瘤的准确性的差异；比较 MSCTA 和 DSA 显示颅内动脉瘤类型的差异；以及 MSCTA 和 DSA 显示大脑中动脉瘤对比。结果：术中发现45个颅内动脉瘤，其中 MSCTA 检查发现41个颅内动脉瘤，准确率为91.11%；DSA 检查发现38个颅内动脉瘤，准确率为84.44%；两种检查方法发现颅内动脉瘤的准确率的差异无统计学意义。MSCTA 检查4例颅内动脉瘤未显示，其中颈内动脉瘤2例，大脑后动脉瘤1例，后交通动脉瘤1例；DSA 检查7例颅内动脉瘤未显示，其中颈内动脉瘤3例，大脑前动脉瘤1例，大脑中动脉瘤1例，前交通动脉瘤2例。DSA 检查不能明确诊断左前交通动脉处造影剂充盈为动脉瘤，MSCTA检查可较清楚的显示前交通处动脉瘤，说明MSCTA检查对DSA 检查有很好的补充作用。结论：虽然 MSCTA 检查在发现颅内动脉瘤的准确率方面与 DSA 检查无显著差异，但对DSA 检查有很好的补充作用。

Methods 37 patients with subarachnoid hemorrhage in our hospital were selected and underwent MSCTA and DSA examination before operation. Com-parison of the accuracy of MSCTA and DSA in the diagnosis of intracranial aneurysms; Comparison of MSCTA and DSA showed differences in the types of intracranial aneurysms; And the comparison of MSCTA and DSA in the middle cerebral artery aneu-rysm. Results 45 of intracranial aneurysms were found in the operation, 41 intracranial aneurysms was found by MSCTA, and the accuracy was 91.11%; 38 of intracranial aneurysms was found by DSA, and the accuracy was 84.44%; There was no sig-nificant difference in the accuracy of the two methods .4 cases of intracranial aneurysms did not show by MSCTA examination, 2 cases of internal carotid artery aneurysm, 1 cases of cerebral aneurysm, 1 cases of posterior communicating artery aneurysm; 7 cases of intracranial aneurysms did not show by DSA examination, 3 cases of internal carotid artery aneurysm, 1 cases of an-terior cerebral artery aneurysm, 1 cases of middle cerebral artery aneurysm, 2 cases of anterior communicating artery aneurysm. DSA examination can’t be a clear diagnosis of left anterior communicating artery angiography agent filling for aneurysm, MSC-TA examination can clearly show the former Department of communications artery aneurysm, MSCTA check for DSA inspection has a very good complementary role. Conclusion Although there was no significant difference in the accuracy of MSCTA and DSA in the diagnosis of intracranial aneurysm, DSA had a very good complementary effect on DSA.
