恶性外周神经鞘膜瘤中基质金属蛋白酶-9及其组织金属蛋白酶抑制剂的表达与临床病理和预后的相关性研究

目的MMP-9、TIMP-1在恶性外周神经鞘膜瘤中的表达与临床病理和预后关系。方法58例恶性外周神经鞘膜瘤的手术切除之癌组织和癌周正常组织，应用免疫组织化学方法检测其MMP-9）和TIMP-1的表达情况，探讨这些因子表达与临床病理和预后关系。结果在58例恶性外周神经鞘膜瘤中：53例MMP-9表达阳性，阳性率为89．7％；35例TIMP-1表达阳性，阳性率为60．3％。这两种蛋白的表达水平与组织类型、组织分化程度和临床分期有统计学相关性（P〈0．001）。MMP-9蛋白表达与五年生存率有影响（P〈0．001）。结论恶性外周神经鞘膜瘤中研究MMP-9、TIMP-1的表达，可能是癌肿发生、生长和浸润转移的重要因素，可作为判断肿瘤恶性程度和预后的重要指标。

Study the Expression of MMP-9 ,TIMP-1 and its Relationship with the Clinical Pathology and Prognosis in Malignant Peripheral Nerve Sheath Tumour s

Objective Study the expression of MMP- 9, TIMP- 1 and its relationship with the clinical pathology and prognosis in Malignant Peripheral Nerve Sheath Tumours（ MPNST）. Methods lmmunohistochemical technique was used to detect the expression of MMP- 9, TIMP- 1 in the carcinoma tissues and normal tissues around the MPNST. The relationship of the expression of these two proteins, together with the clinical pathology and prognosis was further investigated. Results In 58 cases of MPNST patients, positive rate of MMP- 9 is 89.7 % （52/58） and TIMP - 1 is 60.3 % （35/58）. Expressions of these proteins is correlated with clinical pathology and prognosis. There is aiso statistical correlation between the expressions of these protein and tissue types, tumor differentiation, metastasis of lymph node, Dukes stage（P〈0. 001 ）. The expression of MMP - 9 is correlated with five year survival （ P〈0.01 ）. Conclusion The expressions of MMP- 9, TIMP - 1 in MPNST increased may be important factors involved in carcinogenesis, development, invasion and metastasis, which can be used as predictors of malignant behavior and prognosis of MPNST.
