老年食管癌患者癌因性疲乏情况调查及其影响因素探讨

目的:探讨老年食管癌患者癌因性疲乏情况及其影响因素,为临床制定干预措施提供依据。方法:以96例食管癌手术患者为研究对象,采用一般资料问卷、Pi per疲乏量表进行癌因性疲乏评分,分析癌因性疲乏的影响因素。结果:有73例食管癌手术患者发生疲乏,发生率为76.04%,其中整体疲乏56例,行为中度疲乏52例,情感中度疲乏71例,感觉中度疲乏54例,发生率分别为58.33%、54.17%、73.96%、56.25%。化疗后食管癌患者的癌因性疲乏评分高于化疗前,差异有统计学意义(P0.05)。影响疲乏的主要因素为消化道症状、血红蛋白浓度和胸闷气促,影响整体疲乏的主要因素为消化道症状和血红蛋白浓度。结论:老年食管癌手术患者癌因性疲乏的发病率较高,整体疲乏和行为、情感、感觉中度疲乏患者较多,主要影响因素为消化道症状、血红蛋白浓度和胸闷气促,临床护理应针对影响因素提高护理质量。

Investigation into Cancer Related Fatigue Conditions of the Elderly Patients with Esophageal Cancer and Discussion on the Influencing Factors

Objective:To provide the reference for intervention measures establishment in clinic by discussing cancer related fatigue conditions of the elderly patients with esophageal cancer and its influencing factors. Methods:All 96 esophageal cancer patients who accepted the operation were chosen as the subjects, the conditions of cancer related fatigue were graded by using general information questionnaire and Piper fatigued scales, and the influencing factors of cancer related fatigue were analyzed. Results:All 73 patients with esophageal cancer occurred fatigue, the incidence was 76.04%, among them, there were 56 cases with the whole fatigue, 52 cases with moderate behavioral fatigue, 71 cases with moderate emotional fatigue and 54 cases with moderate sense fatige, the incidences were 58.33%, 54.17%, 73.96% and 56.25% respectively. The scales of cancer related fatigue of the patients with esophageal cancer after chemotherapy were notably higher than these before chemotherapy, and the difference had statistical meaning (P<0.05). The main factors of affecting fatigue were symptoms of digestive tract, hemoglobin concentration (HC), choking sensation in chest and shortness of breath, the main factors of affecting the whole fatigue were symptoms of digestive tract and HC. Conclusion:The incidences of cancer related fatigue of the elderly patients with esophageal cancer are higher, there were more patients with the whole fatigue, moderate behavioral, emotional and sense fatigue, main influencing factors are symptoms of digestive tract, HC, choking sensation in chest and shortness of breath, and clinical nursing should be improved according to the influencing factors.
