中药熏洗结合高黏度骨水泥治疗椎体压缩性骨折的临床研究

目的:观察中药熏洗结合高黏度骨水泥对恢复椎体压缩性骨折患者压缩椎体高度的影响。方法:将骨质疏松压缩性骨折患者56例随机分为治疗组和对照组各28例,对照组采用经皮椎体成形术(PVP)治疗,治疗组在此基础上结合中药熏洗治疗。分别在术前及术后3天、6个月对患者进行VAS模拟评分、伤椎高度恢复测量、ODI评分以及患者满意程度调查。结果:术后3天及6个月2组患者VAS评分有显著性差异(P0.05);手术前2组间椎体后凸Cobb角比较差异无统计学意义(P0.05),手术后3天、6个月2组比较有显著性差异(P0.05),对照组椎体后凸Cobb角较治疗组大;ODI评分,手术前2组比较差异无统计学意义(P0.05),手术后3天、6个月2组比较差异有统计学意义(P0.05),治疗组脊柱功能改善更加明显;手术后3天对照组和治疗组满意率分别为85.89%和87.46%,6个月后分别为90.46%和97.21%。结论:PVP结合中药熏洗治疗骨质疏松性椎体压缩性骨折可有效改善患者疼痛症状,有利于恢复压缩椎体的高度,提高患者满意率。

Clinical Study on Herbal Fumigation Combined with High Viscosity Bone Cement in the Treatment of Vertebral Compression Fractures

Objective:To observe clinical effects of herbal fumigation combined with high viscosity bone cement in the recovery of compressed centrum height of the patients with vertebral compression fracture (VCF).Methods:All 56 patients were randomized into the treatment group and the control group,28 cases each group.The patients in the control group adopted percutaneous vertebroplasty (PVP),the treatment group accepted PVP combined with herbal fumigation.VAS scales,measurement of vertebral height recovery,ODI scales and the patients' satisfaction degrees were surveyed before the operation,in three days and six months after the operation.Results:There was significant difference in the comparion of VAS scales in three days and six months after the operation between both groups (P＜0.05);the difference had no statistical meaning in the comparisons of Cobb angle of kyphosis before the surgery between both groups (P＞0.05),there was significant difference in the comparisons between both groups in three days and six months after the operation (P＜0.05),the Cobb angle ofkyphosis of the control group was larger than that of the treatment group,the difference had no statistical meaning in the comparisons of ODI scales between both groups before the operation (P＞0.05),the difference had statistical meaning in the comparison of ODI scales in three days and six months after the operation between both groups (P＜0.05),the improvements of spinal function of the treatment group was more notable,satisfaction degrees of the control group and the treatment group in three days after the operation were 85.89％ and 87.46％,while the degrees of both groups were 90.46％ and 97.21％ in six months after the operation respectively.Conclusion:PVP combined with herbal fumigation in treating the patients with osteoporotic VCF could effectively improve the pain of the patients,help the recovery of vertebral height and raise the patients' satisfaction degrees.
