抵当丸对脑出血后血肿周围组织血流灌注的干预研究

摘    要:目的观察脑出血大鼠血肿周围组织血流的动态变化,探讨抵当丸对血肿周围组织血流灌注的影响。方法雄性Wistar大鼠50只,随机分为对照组、假手术组、脑出血Ⅰ组和脑出血Ⅱ组(治疗组),脑出血组分为12h、24h、72h、7d4个亚组,采用自体血尾状核注入法制备脑出血模型,各时相点应用CT灌注对血肿周围组织血流动力学参数包括局部脑血流量(rCBF)、局部脑血容量(rCBV)及对比剂平均通过时间(MTT),进行定量测量,同时按Bederson评分法进行动物行为评分。结果脑出血Ⅰ组、Ⅱ组rCBF、rCBV明显低于正常对照组、假手术组(P〈0.05),Bederson评分明显高于正常对照组、假手术组(P〈0.05);脑出血Ⅱ组各时相点rCBF、rCBV明显高于脑出血Ⅰ组(P〈0.05),Bederson评分除术后7d低于脑出血Ⅰ组(P〈0.05),其余各时相点无统计学意义(P〉0.05)。结论脑出血后血肿周围组织存在低灌注状态;抵当丸能显著改善脑出血后血肿周围组织血流灌注,对神经功能的改善有促进作用。

Study of Didang Pill on Blood Flow of Tissue around Hematoma in Rats with Intracerebral Hemorrhage

Abstract:Objective To observe the effect of Didang pill on blood flow of tissue around hematoma in rats with intracerebral hemorrhage.Methods Fifty male Wistar rats were randomly divided into four groups:Control group,sham operation group,intracerebral hemorrhageⅠgroup and intracerebral hemorrhage Ⅱ group.Cerebral hemorrhage group was divided into 12 h,24 h,72 h,and 7 d sub-groups.The intracerebral hemorrhage models were established by autologous blood into the caudate nucleus.Regional cerebral blood flow(rCBF),regional cerebral blood volume(rCBV) and contrast agent mean transit time(MTT) were measured quantitatively by CT perfusion.The animal behavior ratings were recorded by Bederson scoring system.Results The rCBF and rCBV in intracerebral hemorrhage groupⅠand groupⅡ were significantly lower than that in control group and sham operation group(P 0.05),and Bederson score was significantly higher than that in control group and sham operation group(P 0.05).The rCBF and rCBV in intracerebral hemorrhage Ⅱ group at each time point were significantly higher than that in cerebral hemorrhage Ⅰ group(P 0.05).Conclusion The brain tissue around hematoma was in the low perfusion after intracerebral hemorrhage state.Didang pills could significantly improve the perfusion of tissue around hematoma and promote improvement of neurological function after intracerebral hemorrhage.
