血浆脑钠肽水平与心衰患者疗效及预后的关系

目的：检测心力衰竭患者治疗前后外周血脑钠肽水平，观察其在心衰患者疗效和预后判断中的作用。方法：收集自2006年2009年间收治的充血性心力衰竭患者87例，在其治疗前后检测外周血脑钠肽水平，并对其进行12个月的随访，以患者因心血管疾病再入院或泵衰竭和心源性猝死为终点事件。结果：心衰组患者入院时脑钠肽水平在823．5±27．6ng／L，显著高于健康对照组的92．6±11．8ng／L；心衰组在治疗两周后外周血脑钠肽水平492．3±18．7ng／L，显著低于其入院时的水平，差异有显著性，P〈0．05；随着心功能分级的升高，其外周血脑钠肽的水平逐渐升高，各组之间差异有显著性，P〈O．05；终点事件发生率在血脑钠肽水平≥500ng／L的患者显著大于脑钠肽水平〈500ng／L的患者，差异有显著性，P〈0．05。结论：对心力衰竭患者治疗前后检测血浆脑钠肽水平有助于对治疗效果和预后进行评价，具有重要的临床价值。

The relationship between the level of brain natriuretic peptide in serum and the curative effects and prognosis of patients with congestive heart failure

Objective.. To detect the level of brain natriuretie peptide in serum before and after treatment in patients with congestive heart failure and observe its application in the curative effects and prognosis of patients with congestive heart failure. Methods： 87 patients with congestive heart failure from 2006 to 2009 in our hospital were collected. The levels of brain natriuretic peptide in serum were detected before and after treatment and carry out 12 months follow--up. The destination event was readmission or sudden death by cardiogenic. Results：The level of brain natriuretic peptide in patients with congestive heart failure was 823.5±27.6 ng/ L, which was significantly higher than that in control. The level of brain natriuretic peptide was 492.3±18.7 ng/L two weeks after treatment, which was notably decreased than before. The level increased gradually with the step up of cardiac functional grading. There were significantly differences between these groups. The incidence rate of destination envent was significantly higher in the patients whose BNP level was over 500ng/L than that in the patients whose BNP level was lower than 500ng/L. Conclnsion：The detection of the level of brain natriuretic peptide before and after treatment in patients with congestive heart failure could conduce to the evaluation of curative effects and prognosis and has crucial clinical value：
