妊娠高血压综合征孕妇胎儿脐动脉血流动力学变化研究

目的：探讨脐动脉血流动力学变化在妊娠高血压综合征孕妇中的研究价值。方法：选取我院2010年4月-2011年5月收治的妊高症患者47例，随机选取本院同期产前检查的正常孕妇47例，作为对照组。47例患者入院后采取利尿、解痉、降压治疗。测量健康孕妇和妊高症孕妇治疗前后的脐动脉收缩期最大血流速度与舒张末期血流速度比值（S／D），搏动指数（PI），阻力指数（RI）。对比分析。结果：妊高症孕妇S／D、PI、RI均高于对照组，对比差异有统计学意义，P〈0．05。35例脐动脉血流指标恢复正常患者的预后明显优于12例脐动脉血流指标未恢复正常的患者，对比差异有统计学意义，P〈0．05。结论：脐动脉血流动力学指标可反映妊高症患者的病情状态，也可为围产儿预后诊断起到指导作用。

A Research on the Fetus Umbilical Artery Hydromechanical Variation for Pregnant Women with Hypertension of Pregnancy Syndrome

Purpose： the research value of umbilical artery hydromechanical variation for pregnant women with hypertension of pregnancy syndrome was discussed. Method： 47 cases of patients with hypertension of pregnancy who were received and cured in our hospital from April 2010 to May 2011 were selected and 48 cases of pregnant women who were diagnosed as normal in the antenatal ex- amination in the same period in our hospital were randomly chosen as the control group. Diuresis, spasmolysis and depressurization treatment were conducted on the 47 patients after they were hospitalized. The ratio （S/D） of maximum blood flow speed during umbili- cal artery systole and that during diastole telophase, pulse rhythm （PI）, resistance index （RI） for healthy pregnant women and those with hypertension of pregnancy were measured and comparative analysis was made. Result： the S/D, PI and RI of pregnant women with hypertension were higher than those of the control group and the comparative difference was of statistical significance （ P 〈 0. 05 ）. The prognosis for the 35 cases of patients whose umbilical artery blood flow indexes returned to normal was obviously superior to the 12 cases of patients with the adverse circumstances. The comparative difference was of statistical significance （P 〈0. 05）. Con- clusion： the hydromechanical index of umbilical artery blood flow can reflect the state of an illness of patients with hypertension of pregnancy, which is of instruction function for the prognosis diagnosis of perinatal infants.
