丹参注射液对重症急性胰腺炎大鼠心脏的保护作用

摘    要:目的探讨丹参注射液对重症急性胰腺炎(SAP)大鼠心肌损伤的保护作用及机制。方法选取雄性SD大鼠60只,随机分为6组,即假手术组,模型组,乌司他丁组(2×104U·kg-1),丹参注射液高、中、低剂量组(15、7.5、3.75 g·kg-1)。实验采用逆行胆胰管灌注5%牛磺胆酸钠溶液复制SAP大鼠模型,造模后3 h,按组别尾静脉注射生理盐水、乌司他丁、不同剂量丹参注射液。6 h后苏木素-伊红(HE)染色法观察各组大鼠胰腺及心肌的病理变化,并检测大鼠心肌细胞Na+-K+-ATP酶活性,qPCR法检测心肌组织肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)mRNA表达量。结果与假手术组比较,模型组的大鼠胰腺、心肌损伤严重,病理评分明显升高(P<0.05),心肌Na+-K+-ATP酶活性明显降低(P<0.05),TNF-α、IL-1βmRNA的表达量明显升高(P<0.05);与模型组比较,各给药组的胰腺和心肌组织病理评分明显降低(P<0.05),丹参注射液高、中剂量组的Na+-K+-ATP酶活性明显升高(P<0.05),丹参注射液高剂量组的TNF-α、IL-1βmRNA表达量明显降低(P<0.05)。结论丹参注射液可能通过提高心肌细胞Na+-K+-ATP酶活性,减少心肌组织中TNF-α、IL-1β的表达,对心肌起保护作用。

Abstract:Objective To investigate the curative effect and mechanism of Danshen injection on severe acute pancreatitis-related myocardial injury. Methods Sixty male SD rats were randomly divided into 6 groups: Sham group,model group,ulinastatin group and Danshen injection (high dose 15 g·kg^-1,midium dose 7.5 g·kg^-1,low dose 3.75 g·kg^-1) . The rats in model group,ulinastatin group and Danshen injection groups were reconstructed via retrograde cholangiopancreatic injection of 5% sodium taurocholate. After 3 h, normal saline, ulinastatin and Danshen injection were injected by the tail vein correspondingly. After 6 h,the pathological changes of pancreas and myocardium were observed,and the Na^+-K^+-ATPase activity,and the mRNA expression of TNF-α,IL-1β and IL- 6 in rat cardiomyocytes were detected. Results Compared with the Sham group,the pancreatic,myocardial injury and pathological scores of the model group were significantly increased (P < 0.05),the activity of Na^+-K^+-ATPase was significantly decreased (P < 0.05), the mRNA expression levels of TNF- α and IL- 1β were significantly increased (P < 0.05). Compared with the model group, the pancreatic and cardiac pathological scores of the ulinastatin group and Danshen injection groups were significantly decreased (P < 0.05). Compared with the model group,the activity of Na^+-K^+-ATPase in the myocardium of Danshen injection medium and high dose groups were significantly increased (P < 0.05),and the mRNA expression of TNF-α and IL-1β of Danshen injection high dose group were significantly decreased (P < 0.05). Conclusion Danshen injection may significantly improve the activity of Na^+-K^+-ATPase in myocardial cells and decrease the expression of TNF-α and IL-1β in mRNA in myocardium and protect the myocardium.
