中医导痰祛瘀法治疗2型糖尿病合并冠心病临床观察

目的观察中医导痰祛瘀法治疗2型糖尿病合并冠心病的临床疗效。方法将104例2型糖尿病合并冠心病患者随机分为2组,对照组52例予盐酸曲美他嗪片治疗,治疗组52例在对照组治疗基础上应用中医导痰祛瘀法治疗,2组均用药6周。观察2组治疗前后血脂指标胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)变化,空腹血糖(FPG)、血压及心率变化,比较2组疗效及并发症发生情况。结果治疗组治疗后TC、TG、LDL-C均较本组治疗前降低(P0.05),HDL-C升高(P0.05);对照组治疗后TG、LDL-C均较本组治疗前降低(P0.05);治疗组治疗后TC、TG、LDL-C均低于对照组治疗后(P0.05),HDL-C高于对照组治疗后(P0.05)。治疗组治疗后FPG、血压及心率均较本组治疗前降低(P0.05),且低于对照组治疗后(P0.05)。治疗组总有效率92.3%,对照组总有效率67.3%,2组总有效率比较差异有统计学意义(P0.05),治疗组疗效优于对照组。治疗组并发症发生率5.7%,对照组并发症发生率17.3%,治疗组并发症发生率低于对照组(P0.01)。结论中医导痰祛瘀法治疗2型糖尿病合并冠心病临床疗效显著,能改善患者血脂指标,降低患者FPG、血压及心率,降低患者并发症发生率,值得临床推广应用。

Clinical observation of guiding phlegm and dispelling stasis therapy of TCM on type 2 diabetes mellitus with coronary heart disease

Objective To observe the clinical effects of guiding phlegm and dispelling stasis therapy of traditional Chinese medicine (TCM) on type 2 diabetes mellitus with coronary heart disease.Methods 102 type 2 diabetes mellitus with coronary heart disease patients were randomly divided into 2 groups.52 cases in the control groups were treated by trimetazidine hydrochloride tablet,and 52 cases in the treatment group were treated by guiding phlegm and dispelling stasis therapy of TCM on the basis of the control group,2 groups were treated for 6 weeks.The changes of fasting plasma glucose (FPG),blood pressure,heart rate and the serum lipid indexes including total cholesterol (TC),triglyceride (TG),high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol (LDL-C) before and after treatment were observed in two group.And the clinical effects and the incidence of complications were compared between the two groups.Results The TC,TG and LDL-C after treatment reduced in the treatment group (P ＜ 0.05),and HDL-C increased (P ＜ 0.05).The TG and LDL-C after treatment reduced in the control group (P ＜ 0.05).The TC,TG and LDL-C after treatment in the treatment group were lower than that in the contml group at the same period (P ＜ 0.05),and the HDL-C was higher (P ＜ 0.05).The FPG,blood pressure and heart rate after treatment reduced in the treatment group (P ＜ 0.05),and which were lower than that in the control group at the same period (P ＜ 0.05).There were statistical differences on the total effective rate between the treatment group (92.3％) and the control group (67.3％,P ＜0.05),and the clinical effects in the treatment group was superior that in the control group (P ＜0.05).The incidence of complications in the treatment group (5.7％)was lower than that in the control group (17.3％,P ＜ 0.05).Conclusions Guiding phlegm and dispelling stasis therapy of TCM has significant clinical effects on type 2 diabetes mellitus with coronary heart disease,can improve the serum lipid indexes,and reduce the FPG,blood pressure,heart rate and the incidence of complications of patients,worthy of clinic application.
