自我决定动机对脑卒中患者运动功能和ADL的影响

目的：探讨自我决定动机在脑卒中患者康复过程中的应用价值;探索＂自我决定中介模型＂在临床康复过程中的作用是否存在中西方文化间的差异。方法：对54例脑卒中患者针刺治疗前、中、后进行一系列与自我决定动机有关的问卷调查和运动功能、日常生活活动的评分。结果：自主性支持对运动功能和日常生活活动（ADL）的改善无影响。自我效能感对ADL有积极的影响（β=0.24,P〈0.05）。结论：本研究结果未能证实＂自我决定中介模型＂的假设,但是研究验证自我效能感对脑卒中患者的运动康复过程的重要性。根据＂自我决定理论＂,自我效能感是影响个体行为的重要动机因素之一。自我效能感的提升增强患者自我决定动机,从而促进他们的康复过程。

Effect of Self-determination Motivation on the Movement Function and Activities of Daily Living of Patients with Stroke

Objective：To explore the effect of self-determination motivation on the rehabilitation of patients with stroke;to study whether there is a cultural difference in terms of the role of self-determination model in patients＇ rehabilitation.Methods：54 participants received 8 weeks＇ acupuncture treatment. They completed self-determination related questionnaires 3 times over the treatment period （i.e.,pre-,mid-,post-treatment）. Movement function and ADL were collected as well. Results：autonomy support did not affect patients＇ movement function and ADL. Sense of competence had significant impact on ADL （β=0.24,P0.05）. Conclusion：The current study did not approve the hypothesis that self-determination model applied to the rehabilitation of patients with stroke.However,results indicated that the sense of competence played an important role in rehabilitation. According to the self-determination theory,sense of competence is a crucial fact that motivates individuals＇ behavior. Increased sense of competence will facilitate patients＇ rehabilitation.
