放射性肺炎辨证分型临床报道分析

目的：探讨放射性肺炎中医病机及常见证型。方法：对符合入选标准的61篇文献共2256例放射性肺炎的中医辩证分型进行统计分析,分析其病机。结果：61篇文献中单证型者占27.0%,二证型者占53.3%,三证型者占19.7%。单证中多见的证型为气阴（津）两虚型占38.4%,热毒阴虚型占20.6%,肺燥津（阴）亏型占16.7%,热毒内蕴型占14.3%,肺络血瘀型及肺肾阴虚型较少见,均占0.05%。二证型中多见的证型依次为热毒血瘀、气阴两虚型占29.1%,气阴两虚、痰瘀互结型占28.5%,热毒伤阴（津）、肺络瘀阻型占25.3%,少见的证型为肺脾气虚、湿阻血瘀型占0.09%,火热伤肺,脾肾亏虚型0.06%,气阴两虚,余毒未尽型0.02%。三证型以热（火）毒犯肺、气阴（津）两亏、痰瘀互结型为主。结论：热（火）毒犯肺、气阴（津）两亏、痰瘀互结为放射性肺炎的常见中医证型,其公认的病机为热毒（火）、肺燥、阴虚、气虚、津亏、血瘀、痰阻。

Analysis of Syndrome Patterns of Radiation Pneumonia in 61 Clinical Reports

Objective：To investigate the TCM pathogenesis and syndrome patterns of radiation pneumonia. Methods：The syndrome patterns of 2256 patients with radiation pneumonia in 61 clinical reports were statistically analyzed. The TCM pathogensis were studied too. Results：In 61 clinical reports,the single syndrome pattern,two syndrome patterns,three syndrome patterns,were 27.0%,53.3%,19.7% respectively.Among the cases with single syndrome pattern,the common syndrome patterns were qi-yin dual deficiency syndrome（29%）,heat torin-yin deficiency syndrome（20.6%）,lung fluid consumption syndrome（16.7%）,heat toxin accumulating in the interior syndrome （14.3%） ,while lung collaterals blood-stasis syndrome and lung-Kidney yin deficiency synderome were less frequent.Amomg the cases with two syndrome patterns,the common syndrome patterns：heat toxin-blood stasis syndrome,qi-yin dual deficiency syndrome（29.1%）,qi-yin dual deficiency syndrome,phlegm-stagnant blood retention syndrome（28.5%）,heat-toxin damging fluid syndrome,lung collaterals blood-stasis syndrome（25.3%）,the rare syndrome patterns：lung-spleen qi deficiency syndrome,damp retention and blood stasis syndrome（0.09%）,fire-heat damging lung syndrome,spleen-Kidney dual deficiency syndrome（0.06%）,qi-yin dual deficiency syndrome,incomplete abatement of toxin syndrome（0.02%）.Among the cases of three syndrome patterns,there were heat-toxin damging lung syndrome,qi-yin dual deficiency syndrome,phlegm-stagnant blood retention syndrome frequently. Conclusions：The common syndromes of radiation pneumonia were heat-toxin damging lung syndrome,qi-yin daul deficiency syndrome,phlegm-stagnant blood retention syndrome.The pathogenesis acknowledged were heat-toxin,lung dryness,yin deficiency,qi deficiency,fluid insufficiency blood stasis and phlegm abstuction.
