无创正压通气治疗慢性阻塞性肺疾病引起的呼吸衰竭疗效分析

目的评价无创正压通气对慢性阻塞性肺疾病（COPD）引起的呼吸衰竭的治疗价值。方法选择符合无创通气指征的COPD急性加重并呼吸衰竭患者,在相关基础治疗的基础上,予无创正压通气（NIPPV）,分析其最终预后与入院时测定指标的相关性。结果无创通气治疗成功率88.5%;无创通气成功者COPD出现呼吸衰竭至开始呼吸机治疗的时间、呼吸频率、体质量指数、APACHEⅡ评分、患者基础肺功能等与未成功者比较有显著性差异。结论NIP-PV治疗COPD并呼吸衰竭疗效确切、安全,可以减少有创通气的使用;适应证可适当放宽。

Analysis of the effect of noninvasive positive pressure ventilation on treatment of respiratory failure of chronic obstructive pulmonary disease

Objective It is to assess the therapeutic effects of noninvasive positive pressure ventilation（NIPPV） on patients with respiratory failure caused by chronic obstructive pulmonary disease（COPD）.Methods Patients in acute exacerbation stage of COPD who developing respiratory failure and who have the indication for NIPPV were selected in this study.The relationship between the prognosis and the indexes observed during hospitalization was analyzed on the basis of the according fundamental therapy combined with NIPPV.Results The success rate with NIPPV was 88.5%;the ventilation time,respiratory frequency,APACHEⅡ score,and the basic lung function were significantly different in NIPPV group compared with the invasive mechanical ventilation group.Conclusion NIPPV is safe and reliable for COPD patients with respiratory failure and can reduce the intubation rate.The indication for NIPPV can be considered to be a little widening appropriate.
