多系统萎缩48例证候特征分析

目的]探讨多系统萎缩的证候特征。方法]依据Gilman等提出的多系统萎缩诊断标准,采集48例多系统萎缩患者初诊时的中医四诊资料,分析证候特征。结果]主要症状和体征是起则头晕,走路不稳,言语慢无底气,言语不清,易怒,汗出异常,阳痿,二便失调,腰腿冷凉乏力,面色萎黄,唇色暗,舌质暗,苔腻,脉滑、细、弱。结论]本病病机特点是五脏亏虚以肝脾肾为著,虚实挟杂贯穿于疾病发展过程中,阳虚气陷为本病之转归;主要证候类型为脾肾阳虚,痰湿内阻;肝肾亏虚,瘀血阻滞;阳虚气陷。

Analysis on syndrome characteristic in 48 patients with multiple systemic atrophy

Objective] To explore the characteristics of syndrome of multiple system atrophy. Methods] According to the diagnosis standard of multiple system atrophy described by Gilman etc. using the cross section method, the Chinese medicine information of 48 firstly diagnosed patients with multiple system atrophy were collected and their characteristics of syndrome were analyzed. Results] The main symptoms and signs included dizziness when standing up, walking unsteadily, talking slowly and weakly, slurred speech, tantrum, sweating abnormally, asynodia, urining and stoolling imbalance, felling the legs and waist debilitation and cool, facial expression etiolate, dark lips and texture of tongue, greasy fur, slide or fine or weak pulse. Conclusion] The feature of pathogenesis of this disease includes liver, spleen, kidney, obvious weak ZANG-organ of five ZANG-organ; excess and deficiency mixing penetrate the process of the development of this disease; yang asthenia and QI sinking are the turnover of this disease. The main syndrome types are: asdthenic splenonephro-yang, phlegmatic hygrosis internal resistance, liver and kidney debility, stagnation of blood stasis; yang asthenia and QI singking.
