中西医结合治疗慢性乙型重型肝炎724例

目的]总结724例慢性乙型重型肝炎的治疗经验,探讨预后影响因素及中医药干预时机。方法]所有病例均采用常规中西医结合保肝、退黄、支持、对症等治疗,部分病例应用人工肝及抗病毒治疗,总结本病的总体及不同分期疗效情况,分析疗效与并发症、生化指标相关性。结果]724例慢性乙型重型肝炎患者总有效率33.7%,其中,早期有效率94.6%;中期有效率65.5%;晚期有效率仅为12.1%;预后与年龄、住院治疗时间、并发症及反映肝细胞损伤坏死程度的血清胆红素、凝血酶原时间、胆碱酯酶、总胆固醇等有关。结论]慢性乙型重型肝炎的有效率较低,目前仍缺乏特效治疗手段,对并发症及病情分期两个环节采取中药有效干预,辅以积极的血制品支持治疗,可能会提高临床疗效。
