益气活血通络汤联合化疗对中晚期非小细胞肺癌生存质量影响随机平行对照研究

[目的]观察益气活血通络汤联合化疗对中晚期非小细胞肺癌生存质量的影响。[方法]使用随机平行对照方法,将61例住院患者随机分为两组,对照组30例根据患者的病理类型及机体状况制定化疗方案,并予以支持治疗。治疗组31例化疗方案同对照组。化疗期间及间歇期使用益气活血通络汤(黄芪30g,太子参、茯苓、白术各15g,薏苡仁30g,当归、丹参各15g,川贝母9g,法半夏、桔梗各12g,杏仁9g,甘草5g)1剂/d,早晚分服。28d为1个周期,连续治疗2个周期为1个疗程(化疗期间及间歇期对症治疗化疗不良反应)。观测KPS评分、体重变化、临床症状、病灶变化、毒副反应。治疗1疗程,判定疗效。[结果]KPS评分,治疗组显著增加(P<0.01),对照组变化不明显(P>0.05),治疗组优于对照组(P<0.01);治疗组体重增加(P<0.01),对照组体重减少(P>0.05),治疗组体重改善优于对照组(P<0.01);临床症状均有不同程度缓解,咳嗽、气急、乏力治疗组缓解优于对照组(P<0.05):痰血、发热、食欲不振组间差异不明显(P>0.05);病灶变化有效率及稳定率两组无明显差异(P>0.05);两组均出现不同程度白细胞、血红蛋白、血小板减少,转氨酶、胆红素、肌酐升高及恶心呕吐,其中白细胞减少、恶心呕吐发生治疗组明显低于对照组(P<0.05)。[结论]益气活血通络汤联合化疗可稳定瘤体,改善临床症状及体能状态、增加体重,减少化疗不良反应发生,提高患者生存质量。

The Effect of Yiqihuoxue Collaterals Soup with Chemotherapy on Quality of Life for Advanced Non-small Cell Lung Cancer-A Randomized Controlled Study

[Objective]Observe the Yiqihuoxue meridians soup combined with chemotherapy on quality of life In advanced non-small cell lung cancer.[Method]Using a random parallel control method,by random number table 61 cases of hospitalized patients were divided into two groups,treatment group(Yiqihuoxue collaterals soup combined with chemotherapy-31 cases) and the control group(chemotherapy alone-30 cases).Yiqihuoxue collaterals soup(Huangqi 30g,Taizishen 15g,Fuling 15g,Baizhu15g,Miren 30g,Danggui 15g,Danshen 15g,Chuanbeimu 9g,Fabanxia 12g,Jiegeng 12g,Xingren 9g,Gancao 5g)was boiled daily,divided into morning and evening use.The two groups were observed before and after the change in Two cycles(a cycle-28d) of treatment through the following aspects: Karnofsky score,weight,Chinese Symptoms Rating Scale,lesion size and the chemotherapy side effects.[Result]KPS score of the treatment group increased significantly(P<0.01),did not change significantly in the control group(P>0.01),The treatment group improved better than the control group(P<0.01).the weight increased in the treatment group,decreased in the control group,The treatment group improved better than the control group(P<0.05).Clinical symptoms in both group had different levels of mitigation,Cough,shortness of breath,fatigue in treatment group improved better than the control group,with significant difference;hemoptysis,fever,loss of appetite had no significant difference between the two groups;Tumor response rate and the disease control rate in treatment group were higher than control group,however,there is no statistically significant difference.In two groups,White blood cells,hemoglobin,platelets reduced,transaminases,bilirubin,creatinine increased,nausea and vomiting occured.Neutropenia,nausea and vomiting in treatment group was significantly lower than the control group.[Conclusion]Yiqihuoxue collaterals soup combined with chemotherapy can stabilize tumor progression to certain extent;by reducing the toxicity of chemotherapy,improving the clinical symptoms,weight and physical state,thereby enhancing the quality of life.
