小针刀加外敷中药治疗膝关节骨性关节炎80例临床观察

目的]观察小针刀加中药外敷治疗膝关节骨性关节炎疗效。方法]将80例膝关节骨性关节炎住院患者,根据X线片及局部按压找到膝关节周围高应力点(多为骨刺形成部位),每次选取4～8个点,麻醉后针刀快速刺入皮肤,进入皮下(避开浅静脉),刀口线与局部肌肉走形方向一致,深度达骨面,做纵形切割,在骨刺的尖部做切开松解术,一般切割3～5刀,粘连严重多切几刀,再横行摆动,感觉刀下松动后再行下一刀。术毕出刀,用纱布压迫止血后创可贴封住刀口2d,避免感染。1次/周,连续治疗3次为1个疗程。每次针刀治疗2d后外敷中药(川乌、草乌各15g,红花、补骨脂、杜仲各20g,川牛膝、伸筋草、威灵仙、川芎各15g)药物研细末,加少许蜂蜜,用生理盐水调成糊状,均匀涂于药布上,外敷患处30min,1次/d。观测临床症状、关节功能。连续治疗2疗程(6周),判定疗效。结果]痊愈58例,显效12例,有效10例,总有效率100.00%。结论]小针刀加外敷中药治疗膝关节骨性关节炎操作简便,不受场所限制,切口小、出血少,不需要缝合,切口不易感染,可明显减轻膝关节疼痛,改善关节活动度,疗效确切,治疗时间短,疗程短,患者易于接受,值得推广。

Objective]To observe a small knife and external application of Chinese medicine treatment of osteoarthritis of the knee efficacy.Method]80 cases(hospitalization) in patients with osteoarthritis of the knee,according to the X-ray and the local press to find the high-stress points around the knee(and often bone spurs form parts),each select four to eight points after anesthesia the needle knife quickly into the skin,into the subcutaneous(avoid the superficial venous),edge line with local muscle go-shaped in the same direction,the depth of the bone surface,to do longitudinal cutting,to do incision lysis tip of the spur,and generally cutting 3 ～ 5 knives,severe adhesion cut several times,then rampant swing,feeling the sword loose and then under the knife.The surgery a knife,Band-Aid to seal the incision with gauze to stop bleeding,2days,to avoid infection.1 times / week,continuous therapy three times for a course of treatment.The every time acupotomology therapy the 2days after topical Chinese medicine(Chuanwu,Caomu 15g,Honghua,Buguzhi,Duzhong 20g Chuanniuxi,Shenjincao,Weilingxian,Chanxiong each 15g) drug research to fine,add a little honey into a paste with saline,evenly on drugs cloth,topical lesion 30min,1 times / day.Observation of clinical symptoms and joint function.Continuous treatment regimen(6 weeks) to determine efficacy.Results]58 cases were cured,12 cases markedly effective in 10 cases,total effective rate was 100.00%.Conclusion]A small knife and external application of Chinese medicine treatment of osteoarthritis of the knee is simple,not place restrictions,small incision,less bleeding,no sutures,incision infection is not easy,can be significantly reduced knee pain,improve joint activity efficacy,short treatment time,a short course of treatment,patients are easy to accept,is worth promoting.
