咳嗽变异性哮喘中医证型的初步探讨

目的:探讨咳嗽变异性哮喘的中医证候分型及临床演变规律。方法:通过历代文献复习及相关研究资料的综合分析,筛选出CVA临床可常见到的62项症状及舌脉象,制定出统一的证候学科研观察表,对265例CVA患者进行了系统的临床观察与研究。结果:初步总结出咳嗽变异性哮喘的核心症状及周边症状的基本特征性表现,并提出了以风邪犯肺为核心证型,其余5个基本证型(寒邪侵肺、热邪蕴肺、肝火犯肺、肺阴亏耗、肺气亏虚)可互为兼夹的证候学动态变化特点及相应的演变规律。结论:风邪犯肺,肺失宣肃是咳嗽变异性哮喘致病的关键所在,疏风治疗必须贯穿疾病的始终。

Study on TCM syndromes of cough variant asthma

To explore the TCM symptoms and its clinic development rule of cough variant asthma(CVA).Methods: On basis of reviewing successive dynasties literature and analyzing relative materials,we screened 62 symptoms,picture of the tongue and pulse tracings relative to CVA,worked out unified clinical observation questionnaire and made systematic clinic observation and study on 265 cases of CVA.Results: We summarized the basic characteristic of core and peripheral symptoms of CVA,and put forward that the core syndrome was wind evil invading lung,and other five basic syndromes were(cold evil invading lung,heat evil accumulating in lung,liver fire invading lung,yin deficiency of lung,qi deficiency of lung).Conclusion: Wind evil invading lung and impaired depurative descending of lung qi is the key to the cause CVA.Dispelling wind should run through whole of disease.
