议恶性血液病的重要治法——温化寒毒法

恶性血液病包括急慢性白血病、淋巴瘤等,对恶性血液病(特别是急性白血病)的病因病机多从热毒内盛、气阴不足、痰瘀互结、脾肾不足等角度阐述,清热解毒、益气养阴、活血化痰、补脾益肾为治疗常用大法,也取得了一定的疗效,然而目前把寒毒作为恶性血液病的病因病机很少论及。砒霜提取物二氧化二砷(As2O3)及雄黄治疗各种恶性血液病已取得了巨大成功,特别是对急性髓系白血病M3的疗效更是获得了世界范围内的认可。然雄黄、砒霜皆属大热、有毒之品,因而部分恶性血液病的病因可认为是寒毒内盛,温化寒毒法应是此类恶性血液病的主要治法。目前,对于骨髓增生异常综合征(MDS)的治疗以温化寒毒法显示出了良效。另外,当前恶性血液病中存在的难点可试着从温化寒毒入手,寻找开发温化解毒之品,以进一步提高恶性血液病的中医治疗水平。

Discussion about an important treatment way of malignant blood diseases,method of warming the cold-toxicity

Malignant blood diseases contain acute leukemia,chronic leukemia,lymphoma and so on.At present,heat toxin,deficiency of Qi and Yin,phlegm and blood stasis,deficiency of spleen and kidney are considered to be the main pathogenesises of malignant blood diseases(especially acute leukemia).Thus,clearing away the heat toxin,nourishing the Qi and Yin,removing the phlegm and blood stasis,reinforcing the spleen and kidney are the primary ways for the treatment of the diseases,and have obtained certain curative effect.However,cold toxin as the etiology of malignant blood diseases has been rarely discussed.We know that arsenic and realgar has achieved great success in treating malignant blood diseases,especially acute leukemia M3 and the success is worldwide.Because arsenic and realgar are hot and poisonous,the etiology of some malignant blood diseases blood can be thought of cold poison and the method of warming cold toxicity should be the main treatment for such malignant blood diseases.In recent years,warming cold poison has gotten good clinical effect in the trearment of myelodysplastic syndromes(MDS).In addition,the means of warming cold poison will overcome some difficulties existing in the treatment.We should develop and look for other Chinese medicines which have the fuction of warming cold poison and the treatment level will be impved in the future.
