不同中医证型中年女性原发性高血压患者的脉图研究

目的观察不同中医证型的中年女性原发性高血压患者的脉图特点,评价其与心脏、血管相关生理病理状态之间的关系,探讨相关的生理病理机制。方法对66例中年女性原发性高血压患者及20例健康中年女性进行脉图检测,比较痰湿壅盛证组、肝火亢盛证组、阴虚阳亢证组与健康对照组之间各脉图时域参数的差异。结果与痰湿壅盛证组和健康对照组比较,肝火亢盛证组及阴虚阳亢证组弦度系数(W1/t)、张力系数(h3/h1)增大(P0.05);与健康对照组、肝火亢盛证组和阴虚阳亢证组比较,痰湿壅盛证组h4/h1及h5/h1值增大(P0.05)。结论不同证型的中年女性原发性高血压患者脉图可反应其心脏、血管的相关功能状态特点;大血管顺应性较差是肝火亢盛证和阴虚阳亢证患者的病理生理基础,肝火亢盛证患者尚伴有明显的心脏射血增强,痰湿壅盛证患者主要存在外周阻力的升高。

Study on pulseogram of different TCM patterns of primary hypertension in middle-aged woman

Objective To observe the characteristics of pulseogram of different TCM patterns of primary hypertension in middle-aged woman, evaluate its relationship with physiological and pathological conditions related to cardiovascular diseases and to explore its physiological and pathological mechanism. Methods The pulseogram detection was carried out among 66 primary hypertension and 20 healthy middle-aged women. We compared the differences of pulseogram timedomain parameters among the four groups： exuberance of wind-damp group, hyperactivity of liver fire group, yin deficiency with yang hyperactivity group and healthy control group. Results Compared with exuberance of wind-damp group and healthy control group, the W1/t and h3/hl were increased in hyperactivity of liver fire group and yin deficiency with yang hyperactivity group （ P 〈 0. 05 ） ; compared with healthy control group, hyperactivity of liver fire group and yin deficiency with yang hyperactivity group, the value of h4/hl and h5/hl were increased in exuberance of wind-damp group （ P 〈 0.05 ）. Conclusion Pulseogram of different TCM patterns Can reflect its characteristics of heart and vascular functions in middle-aged women with primary hypertension ; patients with hyperactivity of liver fire pattern usually complaints with enhanced cardiac ejection. Poor compliance of large vessel is the physiological and pathological basis of liver fire hyperactivity pattern and yin deficiency with yang hyperactivity pattern in primary hypertension. Peripheral resistance was increased in patients of exuberance of wind-damp pattern.
