健脾疏肝煎治疗肝郁脾虚型活动期溃疡性结肠炎的临床研究

目的探讨健脾疏肝煎治疗活动期肝郁脾虚型溃疡性结肠炎的临床疗效。方法将142例患者随机分为治疗组(72例)和对照组(70例),治疗组在口服柳氮磺吡啶片的基础上加服健脾疏肝煎治疗,对照组在口服柳氮磺吡啶片的基础上加用中药安慰剂治疗。两组疗程均为10周,观察中医证候疗效及临床症状与体征评分、肠镜下结肠黏膜病变评分的变化情况。结果①治疗10周后,共有138例患者完成临床观察,治疗组70例,对照组68例。②治疗组和对照组总有效率分别为84.29%、72.06%;组间中医证候疗效比较,差异有统计学意义(P0.05)。③治疗前后组内比较,两组临床症状与体征评分差异均有统计学意义(P0.05);组间治疗后比较,腹痛、腹泻、里急后重评分差异有统计学意义(P0.05),黏液脓血便评分差异无统计学意义(P0.05)。④治疗前后组内比较,两组肠镜下结肠黏膜病变评分差异均有统计学意义(P0.05);组间治疗后比较,肠镜下结肠黏膜病变评分差异有统计学意义(P0.05)。结论健脾疏肝煎治疗活动期肝郁脾虚型溃疡性结肠炎可显著减轻患者的临床症状,提高中医证候疗效,改善结肠镜下黏膜病变。

Clinical study of ＂Jianpi Shugan Decoction＂ for the treatment of ulcerative colitis at active stage of stagnation of liver qi and spleen deficiency pattern

Objective To observe the clinical efficacy of ＂Jianpi Shugan Decoction＂ in treating ulcerative colitis （UC） at active stage of liver qi stagnation and spleen deficiency pattern. Methods One hundred and forty-two cases were randomized into two groups： treatment group in which 72 cases were treated by Sulfasalazine Tablets and ＂Jianpi Shugan Decoction＂, and control group in which 70 cases were treated by Sulfasalazine Tablets and Chinese placebo, with the course of 10 weeks. The TCM syndrome efficacy, clinical symptoms, scores of signs improvement, and colonic mueosa lesion score ureter colonoscopy were observed. Results （~ After ten weeks of treatment, there were 138 cases finished the clinical observation, with 70 cases in treatment group and 68 cases in control group. （~ The overall eftective rate was 84.29% in the treatment group and 72.06% in the control group, with a difference between the ｜wo groups （P 〈0.05）. （~ In each gn）up, there were significant differences in clinical symptoms and scores of signs improvement between before and afler treatment （P 〈0.05）; after treatment, there were significant differences in clinical symptoms and scot, s of signs improvemenl （abdominal pain, diarrhea, tenesmus） between the two groups （P 〈 0.05 ） , and no significant dift~t~nce was found in bloody mucopurulent stool （P 〉 0.05 ）. In each group, there was significant difference in colonic mueosa lesion score under colonoscopy between bell）re and after treatment （P 〈 0.05 ） ; after treatment, there was significant difference in colonic mucosa lesion score between the two groups （ P 〈 0.05）. Conclusion ＂Jianpi Shugan Decoction＂ can relieve the clinical symptoms of UC, improve TCM syndromes, and improve colonic mucosa lesion. Its curative effect is better than western medicine.
