“通荣”理论探析糖尿病周围神经病变

糖尿病周围神经病变是糖尿病最常见的慢性并发症,是导致糖尿病患者足部溃疡、感染及坏疽最主要的原因之一。通过分析中医学“通荣”理论的内涵,认为构成人体的微观物质、结构为保持生理状态而运动不息的特性谓之“通”,人体内精微物质的濡养功能谓之“荣”,糖尿病周围神经病变病程中表现出的“凉、麻、痛、痿”等症状皆可以人体“通荣”之性失于常态所解释,故而从“通荣”角度再识糖尿病周围神经病变,提出“通荣失常”实为糖尿病周围神经病变的核心病机,进而提出“通荣并调”作为糖尿病周围神经病变的治疗原则,可将各家理论合理归纳,方便临床掌握应用。

"Tong Rong"theory on diabetic peripheral neuropathy

As the most common chronic complication of diabetes,diabetic peripheral neuropathy is one of the main causes of foot ulcers,infections and gangrene of diabetes.Based on the analysis of the connotation of the theory of"unblocking and nourishing"in Traditional Chinese Medicine,it is believed that the constantly dynamic characteristics of human body's micro materials and structure are called"unblocking"and the function of cultivation for human body's micro materials is called"nourishing".The"cold,numb,pain,flaccidity"in the course of diabetic peripheral neuropathy can all be explained by the abnormal"unblocking and nourishing"in the human body.Therefore,from the perspective of"unblocking and nourishing"to recognize diabetic peripheral neuropathy,it is proposed that"disharmony of unblocking and nourishing"is the core pathogenesis of diabetic peripheral neuropathy,and then"regulate both unblocking and nourishing"is proposed as the treatment principle of diabetic peripheral neuropathy.The final goal is to summarize the theories of various schools for the convenience of clinical application.
