加味柴芩承气汤治疗妊娠合并急性胰腺炎28例临床分析及文献复习

目的：观察加味柴芩承气汤治疗妊娠合并急性胰腺炎（APIP）的疗效。方法：回顾性分析28例APIP采用加味柴芩承气汤治疗的疗效，同时选取中国期刊全文数据库关于西医治疗APIP的文献43篇，共615例患者。结果：本组19例MAP好转出院；9例SAP1例死亡，2例行剖宫产术，其中1例同时行剖腹探查术，2例均好转出院；本组孕妇病死率3．57％；胎儿死亡3例，死亡率为10．71％。文献中615例患者，孕妇死亡36例，病死率5．85％。胎儿死亡94例，死亡率15．16％。采用加味柴芩承气汤治疗址口的孕妇及胎儿死亡率均低于西医治疗。结论：加味柴芩承气汤治疗APIP有一定疗效。

Clinical Analysis and Literature Reviews of Jiaweichaiqinchengqi Decoction in Treating Acute Pancreatitis in Pregnancy

Objective： To explore the therapeutic effects of Jiaweichaiqinchengqi decoction （JWCQCQD） in treating acute pancreafitis in pregnancy （APIP） . Methods： The clinical data of 28 cases of APIP treated with JWCQCQD were retrospectively analyzed. There were 19 cases of mild acute pancreatitis （MAP） and 9 cases of severe acute pancreatitis （SAP） . Meanwhile 43 literatures about western medicine treating 615 cases of APIP were selected in Chinese periodical full-text database. Results： All the patients with MAP were cared. In the 9 cases of SAP, one case was dead, and two were operated cesarean section with one was operated exploratory laparotomy simultaneously. The maternal motality of 28 cases was 3.57%. Three fetuses were dead, and the fetal mortality was 10.71%. Of the 615 cases in literatures, 36 cases were dead, and the maternal mortality was 5.85% ; meanwhile, 94 fetuses were dead, and the fetal mortality was 15.16%. Using JWCQCQD to treat APIP, the mortality of fetus and pregnant women were lower than those when using western medicine to treat APIP. Conclusion： JWCQCQD has certain curative effect on APIP.
