针刀治疗神经根型颈椎病的症状体征比较研究

目的:研究后侧入路针刀治疗神经根型颈椎病的可行性及有效性,并探讨针刀治疗神经根型颈椎病的作用机理,为临床防治提供依据.方法:选择神经根型颈椎病60例,随机分为两组,即针刀治疗神经根型颈椎病30例(治疗组),单纯推拿治疗神经根型颈椎病30例(对照组).分别治疗3个疗程,随访3个月判定疗效.对两组前后症状、体征积分变化及临床疗效进行分析比较,并探析针刀治疗神经根型颈椎病的相关机制.结果:治疗组与对照组治疗后症状、体征比较,P＜0.05;治疗组总有效率为96.7%,对照组总有效率为80%,两组比较P＜0.05;上述数据说明治疗组与对照组疗效有显著性差异.结论:(1)针刀疗法是临床治疗神经根型颈椎病一种可行、有效的方法.(2)针刀治疗神经根型颈椎病的作用机理是:能够解除颈肩软组织痉挛、松解粘连;减轻颈部神经及关节囊等炎症水肿,消除无菌性炎症;改善颈肩微循环、促进神经细胞再生,增快组织修复;增强颈肩部免疫作用,调整神经功能.

Symptoms Physical Sign Compare Study on Needle-Scalpel Treatment of Radicular Cervical Vertebrae Diseases

Objective:To study feasibility and effectiveness of needle-scalpel treatment for radicular cervical vertebrae diseases with posterior-route operations,and to examine the effect mechanism of needle-scalpel treatment for radicular cervical vertebrae diseases,with an aim to providing bases to clinical applications.Methods:To select 60 cases of radicular cervical vertebrae diseases,and to divide them into two groups at random,that is,30 cases for needle-scalpel treatment for radicular cervical vertebrae diseases(treatment group),and the other 30 cases for sole massage treatment(contrast group).To proceed with 3 courses of treatment respectively and to estimate the cure within the subsequent 3 months.To analyse and compare the symptoms,physical sign changes,and effectiveness,and to explore and analyse the mechanism of needl-scalpel treatment for radicular cervical vertebrae diseases.Results:After treatment the comparison result of symptoms and physical signs between treatment group and contrast group is P<0.05.The overall effectiveness of the treament group is 97.7%,while the contrast group is 80%.The results companed,we can conclude P<0.05.These data show that there is a notable differential between the two groups.Conclusions:(1)Needle-scalpel treatment is a feasible and effective method in the clinical cure of radicular cervical vertebrae diseases.(2)The effect mechanism of needle-scalpel treatment for radicular cervical vertebrae diseases is:to be able to eradicate neckshoulder soft tissues spasm and dissolve adhesion,to relieve inflammation and dropsy of neck nerve and articular capsule and eliminate aseptic inflammation,to improve the microcirculation and around the neck and shoulder,to help the regeneration of nerve cells and accelerate the restoration of tissues,and to strengthen the immunity of the neck and shoulder and fine-tune the nerve functions.
