激素性股骨头坏死中医病机学实验研究

目的:探讨激素诱导的股骨头坏死中医发病机理.方法:用外源性糖皮质激素(醋酸氢化泼尼松)诱导家兔股骨头坏死模型为对象,33只雄兔随机分为三组,对照组、模型组、模型 右归饮组(右归饮组),动态观察下丘脑--垂体--肾上腺皮质(HPA)轴形态和功能,以及性激素水平变化.结果:模型组HPA轴出现形态萎缩功能抑制状态,表现为垂体、肾上腺重量减轻(P＜0.01),垂体前叶ACTH细胞减少及血浆促肾上腺皮质激素(ACTH)含量降低(P＜0.05),肾上腺皮质萎缩及血浆皮质醇(CORT)降低(P＜0.01).性激素水平紊乱,表现为雌二醇(E2)升高(P＜0.01),睾酮(T)降低(P＜0.05),E2/T明显紊乱升高(P＜0.01).右归饮能有效拮抗激素诱导的股骨头坏死HPA轴从形态到功能的抑制状态,纠正性激素水平的紊乱.结论:激素诱导的股骨头坏死中医病机学属于肾阳虚证.

Experimental Research of the TCM Pathogenesis in the Steriod-induced Femoral Head of Necrosis

Objective:An experimental model of Steroid-induced Avascular Necrosis of the Femoral Head (ANFM) was established to investigate the TCM Pathogenesis.Methods:33 rabbits were peformed model there groups.INFH rabbit model was established by injecting Hydroprednison.As an exact inducator for the Hypothalamic-Pituitary-Adrenal (HPA) axis andthe level of sex hormone.Results:Both the morphology and the function of the HPA axis were inhibited significantly in INFH rabbits:Pituitary and Adrenal weight decreased (P<0.05),adrenal cortex was atrophy with the amount of plasma CORT decreaded (P<0.01).The level of sex hormone disordered:E_2 increaded (P<0.01).T decreaded (P<0.05) and E_2/T significantly increased.After administered You Gui (YG) to the experimental group,it was found that YG antagonised the inhibition of the HPA axis,alter the disordered sex hormone.Conclusions:In this experiment,INFH belonged to the symptom of Deficiency of Kidney-Yang.
