活血愈溃汤联合双歧杆菌四联活菌片治疗Hp阳性消化性溃疡临床观察

目的:观察活血愈溃汤联合双歧杆菌四联活菌片治疗幽门螺杆菌(Hp)阳性消化性溃疡的临床疗效及其对血浆胃动素(MTL)、生长抑素(SS)以及炎症因子水平的影响。方法:将Hp阳性消化性溃疡患者85例随机分为观察组43例、对照组42例,2组均给予双歧杆菌四联活菌片治疗,观察组在此基础上给予活血愈溃汤治疗,4周为1疗程,治疗1疗程,观察临床疗效,监测MTL、SS以及炎症因子水平。结果:观察组临床治愈率、症状缓解率、Hp清除率均高于对照组,复发率低于对照组,差异均有显著性意义(P0.05)。治疗后观察组MTL低于对照组,而SS高于对照组,差异均有显著性意义(P0.05)。观察组治疗后白细胞介素-2(IL-2)、IL-6、IL-8及肿瘤坏死因子-α(TNF-α)均低于对照组,差异均有显著性意义(P0.05)。结论:活血愈溃汤联合双歧杆菌四联活菌片治疗Hp阳性消化性溃疡安全、有效、复发率低,其作用机制可能与其促进SS分泌,抑制MTL及相关炎症因子分泌及释放有关。

Clinical Observation of Huoxue Yukui Decoction Plus Bifidobacterium Tetravaccine Tablets for Treatment of Hp-positive Peptic Ulcer

Objective： To explore the clinical efficacy of Huoxue Yukui Decoction plus Bifidobacterium Tetravaccine Tablets for the treatment of Helicobacter pylori（Hp）-positive peptic ulcer, and to observe its effect on the levels of plasma motilin （MTL）, somatostatin （SS） and inflammatory factors. Methods： Eighty-five Hp-positive peptic ulcer patients were randomly divided into observation group （n=43） and control group（n=42）. Both groups were given Bifidobacterium Tetravaccine Tablets orally, and additionally the observation group was given Huoxue Yukui Decoction. Four weeks constituted one treatment course. After one treatment course, the clinical efficacy was evaluated in the two groups, and the plasma MTL, SS and inflammatory factors levels were also monitored. Results： Observation group had higher clinical cure rate, symptom relief rate and Hp clearance rate, and lower recurrence rate than the control group, the difference being significant （P 〈 0.05）. After treatment, MTL level was lower, SS was higher, and levels of inflammatory factors such as IL-2, IL-6, IL-8 and TNF-α were lower in the observation group than those in the control group （P 〈 0.05）. Conclusion： Huoxue Yukui Decoction plus Bifidobacterium Tetravaccine Tablets is effective, safe and with low recurrence rate for the treatment of Hp-positive peptic ulcer, and its mechanism may be related to promoting SS secretion, inhibiting the secretion and release of MTL and related inflammatory cytokines.
