金匮肾气丸对异丙肾上腺素致心室重构大鼠神经体液因子的调节

摘    要:目的:研究金匮肾气丸对异丙肾上腺素致大鼠心室重构相关神经体液因子和形态学的改变.方法:异丙肾上腺素连续3 d以20,10,5 mg· kg-1递减皮下注射,3 mg· kg-1维持7d制备大鼠心室重构模型,造模成功后分别用依那普利(2 mg·kg-1),金匮肾气丸(7.5 g·kg-1)ig给药5周及15周,从血浆中神经内分泌因子和形态学进行疗效比较研究.结果:血浆中神经内分泌因子的检测:5,15周时模型组血浆中心房钠尿肽(ANP)含量均高于正常组;5周时与模型组比较,金匮肾气丸组的ANP含量均较低(P<0.05);15周时,金匮肾气丸组的ANP、脑尿钠肽(BNP)含量均降低(P<0.05).5周模型组的血清去甲肾上腺素(NE)浓度高于正常组,但无显著性差异,15周模型组的NE显著高于正常组(P<0.05);5,15周时,依那普利组和金匮肾气丸组的NE与模型组比较均无显著性差异.5,15周,模型组的血清血管紧张素Ⅱ(AngⅡ)浓度与正常组比较均无显著性差异.心肌病理形态学观测结果:5周时,模型组有较多成纤维细胞和新生毛细血管,肉芽组织较明显,15周时,模型组呈现广泛纤维化,形成疤痕组织;5,15周时,金匮肾气丸组的心肌细胞均有一定程度的病理变化,但较模型组而言均有改善.结论:金匮肾气丸能够改善异丙肾上腺素致大鼠心室重构过度激活的神经体液水平,但这种改善作用在晚期时较为明显.

Abstract:Objective:To observe the effect of Jinkui Shenqi Wan on the cardioventricular reconstruction in rat induced by Isoproterenol(ISO).Method: The rats were injected subcutaneously by 20,10,5 mg·kg-1 ISO for 3 days and then 3 mg·kg-1 ISO as maintenance dose for 7 days to build the rat model of cardioventricular reconstruction,Enalapril and Jinkui Shenqi Wan were given to the rats for 5 and 15 weeks as interventions and after that,stained the myocardium with Hematoxyln Eosin(HE) staining method and observed the myocardial morphologic changes by light microscope.The density of atrial natriuretic peptide(ANP),brain natriuretic peptide(BNP),norepinephrine(NE) and angiotensinⅡ(AngⅡ) in blood serum were detected by method of ELISA.Result: The results of neuroendocrine factors in blood serum: at 5 weeks,compared with model group,the density of ANP of Jinkui Shenqi Wan group was significantly lower(P<0.05).At weeks 15,compared with model group,the density of ANP and BNP of Jinkui Shenqi Wan group decreased(P<0.05).The density of NE in blood serum of model group at week 5 was higher than normal group,but with no significant difference,while that of model group at week 15 was significantly higher than normal group(P<0.05).At week 5 and 15,NE in the treat groups was no significant difference compared with model group.Both at weeks 5 and 15,there was not significant difference in the density of AngⅡin blood serum between model group and normal group.The results of pathomorphologic observation on myocardium: At week 5,the model group had many collagenoblast and neogenesis micrangium,while the collagenoblasts degenerated into fibrocytes,widespread fibrosis appearanced and the granulation tissues changed into scar tissues at week 15.Both at week 5 and 15,the myocardial cells in enalapril group and Jinkui Shenqi Wan have some extents of pathological changes,but it was less than model group.Conclusion: Jinkui Shenqi Wan has some therapeutic effects on cardioventricular reconstruction by suppressing the over-expression neuroendocrine factors both in earlier period and advanced stage of rat induced by isoproterenol.However,this effect is more obvious for advanced stage of cardioventricular reconstruction.
