针刺内关对心肌缺血-再灌注损伤家兔CGRP和PGE2的影响

摘    要:目的:对比观察电针内关与心肌缺血预处理对心肌缺血-再灌注损伤家兔心肌组织中降钙素基因相关肽(CGRP)及前列腺素E2(PGE2)的影响,以探讨针刺内关抗心肌缺血的作用机理。方法:健康家兔46只,随机分为5组,即假手术组、缺血-再灌注模型组、缺血预处理组、电针内关组、电针足三里组;结扎家兔冠脉左前降支造成心肌缺血模型,以RM46多导生理仪持续监测心电,用硝基四氮唑兰染色测量心肌梗死范围,用放射免疫法检测家兔心肌组织CGRP、PGE2含量。结果:结扎冠脉左前降支后,心梗范围扩大;除预处理组外,其余各组与假手术组比较均有显著性差异,而假手术组、预处理组和针内关组与模型组比较均有明显差异;CGRP和PGE2水平模型组明显低于假手术组、预处理及针内关组(P<0.05或P<0.01)。结论:心肌缺血预处理与电针内关对缺血-再灌注心肌均有明显的保护作用,其作用机制可能与CGRP、PGE2的参与有关。

Abstract:Objective:To probe into the mechanism of electroacupuncture at Neiguan point against myocardial ischemia through observing the effect of acupuncturing Neiguan point and ischemic preconditioning (IP) on myocardial calcitonin geng-related peptide (CGRP) and prostaglandin E2 (PGE2) in rabbits of myocardial ischemic reperfusion injury. Methods:Forty-six healthy rabbits were divided into fake operating group, ischemic reperfusion model group, IP group, electroacupuncture (EA) at Neiguan point group, EA at Zusanli point group. The model of myocardial ischemia was induced by ligaturing left anterior descending branch of coronary artery. The ECG was monitored by using RM46 multilead physiological apparatus. The infarct size was measured by means of NBT dye. The contents of myocardial CGRP and PGE2 was examined by means of radio-immunity method. Results:After ligaturing the coronary artery, the infarct size in all groups except the IP group had significant difference compared with the fake operating group. Moreover the infarct size of both IP group and EA at Neiguan point group had significant difference compared with the model group. The contents of myocardial CGRP and PGE2 in fake operating group and in IP group as well as in Neiguan point group were all significantly higher than in model group (P<0.05, P<0.01, respectively). Conclusion:Both myocardial ischemic preconditioning and EA at Neiguan have significant protective action on ischemic reperfusion myocardium, which is possibly related to participation of CGRP and PGE2.
