温针灸配合中药治疗输卵管不通不孕症研究

目的：观察温针灸配合中药通管汤治疗输卵管不通不孕症的机理。方法：将60例患者随机分为温针灸配合内服中药组（治疗组）与内服中药组（对照组）,每组各30例。治疗组针灸取穴为子宫、气海、三阴交、合谷、太溪、中极。中药内服为通管汤（方由穿山甲、皂角刺、赤芍、丹参、桃仁等活血化瘀药物组成）,根据辨证加减。对照组只服用中药通管汤。2组患者分别于治疗前和治疗后于月经期抽取静脉血测定全血高切变粘度和纤维蛋白原2项参数,治疗前后卵泡期测定双侧卵巢动脉搏动指数和阻力指数,并对治疗前后2组测定参数进行比较,从而探讨温针灸配合中药治疗输卵管不通不孕症的机理。结果：治疗组总有效率为93．3％,对照组总有效率为40％,两纽比较有显著性差异（P〈0．05）;对照组血流变学指标治疗前后比较无显著性差异（P〉0．05）;卵巢动脉搏动指数及阻力指数治疗前后比较无显著性差异（P〉0．05）。治疗组治疗后血流变学指标显著下降,与治疗前相比有显著性差异（P〈0．05）;卵巢动脉搏动指数及阻力指数在治疗后显著下降,与治疗前相比有显著性差异（P〈0．05）。结论：温针灸配合中药治疗输卵管阻塞性不孕症疗效确切,并优于单纯内服中药治疗。此种疗法是通过改善患者血流变学指标并改善患者卵巢动脉的搏动指数和阻力指数,从而改善炎性输卵管周围的供血情况而达到消除炎症促进输卵管再通目的的。

Clinical study on oviduct blocking treated with acupuncture and moxibustion and drinking "Tong guan tang"

Objective:To investigate the efficacy of treating oviduct blocking with acupuncture and moxibustion and drinking "Tong guan tang",and explore the possible mechanism associated with the effect.Methods:Sixty patients of oviduct blocking were randomly divided into two groups,30 cases in the tested group and 30 cases in the controlled group.Observe the amelioration of clinical symptoms and signs after treatment and changes in the bloodstreams of a.ovarica with transvaginal color Doppler(TVCD) ultrasonic examination and amelioration of blood viscosity,then explore its effective mechanism.Results:The symptoms and signs of the tested group have been significantly improved after treatment,there was remarkable difference between the two groups(P<0.05).The total efficacy of tested group was significantly higher than the controlled group(P<0.05).A.ovarica bloodstream had been greatly improved,artery blood flow parameters(PI,RI)in the test group had significant difference from the controlled group(P<0.05).The blood viscosity of tested group had a remarkable amelioration than the controlled group(P<0.05).Conclusion:Acupuncture and moxibustion has preferable efficacy on oviduct blocking,the effective mechanism may be due to the improvement of a.ovarica bloodstream and blood viscosity.
