针刺眼周三穴及风池穴对视神经萎缩患者视觉诱发电位的影响

目的观察眼周三穴（睛明、承泣、上明穴）联合风池穴针刺治疗对视神经萎缩患者视觉诱发电位（VEP）的影响。方法前瞻性随机对照研究。视神经萎缩患者39例，随机分为试验组20例31只眼，对照组19例29只眼。对照组根据辨证给予口服中药汤剂，神经营养支持疗法，试验组在对照组治疗基础上，针刺睛明穴、承泣穴、上明穴、两侧风池穴，观察视觉诱发电位、视力和视野变化。结果治疗4周后2组VEP的P100或P1波潜伏期均改善，试验组振幅N75-P100或N1-P1提高明显优于对照组，差异有统计学意义（P〈O．05）。综合疗效试验组总有效率为81％，对照组总有效率为52％，试验组的治疗效果优于对照组（P〈0．05）。结论眼周三针联合风池穴针刺治疗视神经萎缩有一定的临床疗效。针刺治疗作用的机理可能与改善视神经纤维的传导速度．并且增强视觉冲动的兴奋强度有关。

Impact of puncturing local acupoints combined with body acupoints on visual evoked potential in optic atrophy patients

OBJECTIVE To research and discuss the effect of puncturing three local acupoints around eyes combined with two body aeupoints on visual evoked potential in optic atrophy patients. METHODS Prospective randomized controlled study on 39 patients who met inclusion criteria of optic atrophy was applied. The patients were randomly divided into experimental group, 20 cases 31 eyes, and control group, 19 cases 29 eyes. Then, the control group accepted treatment of decoction through differentiation as well as maintaining therapy of nerve nutrition support. Meanwhile, except basic treatment of controlling group, experimental group received acupuncture treatment, puncturing local Jingming, Chengqi and Shangming combined with Fengchi aeupoints. The observed indicators were visual acuity, vision and VEP. RESULTS As to VEP Pl00 or P1 peak latencies, both sets improved after four weeks treatments. As toVEP amplitude NTs-P100 or N1-P1, the experimental group improved better than controlling group, moreover, it revealed significant statistical difference, P〈0.05. The total efficiency of experimental grouo was 81%, and 52% of controlling group. Thus, the clinical efficiency of experimental group was significantly better than that of controlling group, P〈0.05. CONCLUSIONS Puncturing local acupoints around eyes combined with Fengchi to treat patients with optic atrophy, the clinical efficacy was reliable, and there was better clinical value. The mechanism of acupuncture therapy maybe associated with improving nerve fiber conduction velocity, and enhancing the strength of the visual excitement impulse.
