强肝消脂饮对非酒精性脂肪肝血清瘦素水平及胰岛素抵抗指数的影响

目的:观察强肝消脂饮对非酒精性脂肪肝患者血清瘦素水平及胰岛素抵抗指数的影响。方法:将120例患者随机分为两组:强肝消脂饮治疗组80例,多烯磷脂酰胆碱胶囊对照组40例。两组患者治疗前后分别检测肝功能(ALT、AST)、血脂(CHOL、TG)、血清瘦素、空腹血糖及胰岛素、肝脏CT,并计算胰岛素抵抗指数。结果:在治疗结束时,治疗组ALT、AST明显下降,与治疗前比较有显著统计学意义(P0.01),与对照组比较有明显疗效优势(P0.05);治疗组CHOL及TG亦呈下降趋势,与治疗前比较提示有统计学差异(P0.01),疗效与对照组无统计学差异(P0.05)。分析治疗组治疗前后血清瘦素及胰岛素抵抗指数变化发现,强肝消脂饮治疗可显著降低NAFLD患者血清瘦素水平并显著改善胰岛素抵抗状态(P0.05),其改善作用明显优于对照组(P0.05)。结论:提示强肝消脂饮治疗可通过降低血清瘦素水平、改善胰岛素抵抗,增加肝细胞对胰岛素的敏感性,从而调节血脂代谢,达到治疗NAFLD患者的作用。

Effect of Qianggan Xiaozhi Yin on serum leptin and Insulin Resistance of Nonalcoholic Fatty liver Disease

Objective：To observe the effects of the therapy with Qianggan Xiaozhi Yin on serum leptin and insulin resistance of the patients with Nonalcoholic fatty liver diseas（ NAFLD）. Methods：120 patients had been randomly divided into twogroups：treatment group and control group. 80 cases in treatment group had been treated by Qianggan Xiaozhi Yin; 40 patients in control group had been treated with polyene phosphatidyl choline. The contents of ALT？？AST？？CHOL？？TG？？LP？？FBG and INS in serum of all patients were measured before the treatment and at the end of treatment. HOMA-IR was calculated and the imaging change of the hepatic tissue was observed by computed tomography. Results：At the end of the treatment,HOMA-IR and the content of ALT？？ AST？？CHOL？？TG？？LP in treatment group had been redused significantly. The states of hepatic tissue had been improved in treatment group. The therapeutic effect in treatment group was batter than that in control group. Conclusion：Qianggan Xiaozhi Yin can effectively treat nonalcoholic fatty liver disease by improving insulin resistance and decreasing serum leptin,increase the sensibility of the hepatic cells on insulin and adjust the levels of blood lipid.
