中医药对行机械通气的慢性呼吸衰竭合并胃肠功能障碍的临床观察

目的:观察在西医治疗基础上加用健脾益气,宣肺化痰中药治疗慢性呼吸衰竭合并胃肠功能障碍患者,为制定中西医结合治疗方案奠定基础。方法:对照组35例患者采用西医常规治疗,治疗组37例患者在对照组的基础上加用健脾益气,宣肺化痰中药。观察治疗前、第7、14、21天的胃肠功能评分、腹内压、肠鸣音、排便次数、脱机成功率。结果:中西医结合治疗明显降低患者的胃肠功能评分和腹内压,增加患者的肠鸣音和排便次数,改善胃肠功能,提高患者的脱机成功率(均P<0.05)。结论:健脾益气,宣肺化痰中药具有明显的临床疗效。

Clinical Observation of Traditional Chinese Medicine in the Treatment of Chronic Respiratory Failure with Gastrointestinal Dysfunction During Mechanical Ventilation

Objective:To observe the clinical effect on treating chronic respiratory failure with gastrointestinal dysfunction during ventilation with western medicine routing treatment plus traditional Chinese medicine(TCM) therapy of strengthening the spleen and replenishing Qi,dispersing lung Qi and dissipating phlegm.Also to lay a foundation for integrated traditional Chinese and western medicine treatment of it.Methods:Thirty five cases of Control Group received western medicine routine treatment,while thirty seven cases of Treatment Group received both western medicine routine treatment and differentiation which was given TCM therapy of strengthening the spleen and replenishing Qi,dispersing lung Qi and dissipating phlegm.Such values as gastrointestinal motility score,gurgling sound,stool frequency,intra-abdominal pressure and success ratio of weaning of mechanical ventilation were compared before therapy,on the 3rd,7th,14th and 21st day after therapy.Results:Treatment of integrated traditional Chinese and western medicine treatment obviously decreased intra-abdominal pressure,restore gurgling sound,increasd stool frequency therapy,improved gastrointestinal motility and the success ratio of weaning of mechanical ventilation(P<0.05).Conclusion:TCM of strengthening the spleen and replenishing Qi,dispersing lung Qi and dissipating phlegm is quite effective on treating chronic respiratory failure with gastrointestinal dysfunction during mechanical ventilation
