柴芍六君子汤加减对肝郁脾虚型慢性乙型肝炎免疫介导发病机制的干预研究

目的:柴芍六君子汤加减对肝郁脾虚型慢性乙型肝炎的疗效以及免疫介导发病机制的干预研究。方法:收集60例肝郁脾虚型慢性乙型肝炎患者,随机分为对照组和治疗组,对照组给予常规内科治疗(包括常规保肝治疗、抗病毒治疗等),其中治疗组30例,在常规内科治疗基础上予柴芍六君子汤加减的中药汤剂,观察T辅助细胞(Th1、Th2)、T淋巴细胞(CD+4、CD+8)、HBV-M、HBV-DNA、肝功能等。结果:柴芍六君子汤加减可有效治疗肝郁脾虚型慢性乙型肝炎,主要表现在改善肝功能,降低乙肝表面抗原定量水平,降低病毒载量水平有关,其作用机制可能与下调T辅助细胞(Th1、Th2)、T淋巴细胞(CD+4、CD+8)有关。结论:柴芍六君子汤加减治疗肝郁脾虚型慢性乙型肝炎,可能与其能调节患者免疫功能有关。

Study of Chaishao Liujunzi Decoction on Immune Mediated Pathogenesis of Liver Stagnation and Spleen Deficiency Type~ Chronic Hepatitis B

Objective ：To study the intervention of Chaishao Liujunzi Decoction on immune mediated pathogenesis of liver stag- nation and spleen deficiency type chronic hepatitis B. Methods ：60 patients of liver stagnation and spleen deficiency type chronic hepatitis B were collected and then randomly divided into treatment group and control group. The control group only received con- ventional internal medicine treatment （including the conventional treatment of hepatoprotive and antiviral, etc）. The treatment group were given Chaishao Liujunzi Decoction besides conventional internal medicine treatment. The helper T cell （Thl, Th2） ,T - Lymphocytes （ CD4^＋ , CD8^＋ ）, HBV - M, HBV, DNA and liver function were observed. Results ： Chaishao Liujunzi Decoction was effective in the treatment of liver stagnation and spleen deficiency syndrome of chronic hepatitis B. The effect was mainly in the improvement of liver function, reducing the hepatitis B surface antigen quantitative level and viral load level. The mechanism may be related to the down - regulation of helper T cells （ Thl, Th2） and T - Lymphocytes （ CD4^＋ , CD8^＋ ）. Conclusion： Chaishao Liu- junzi Decoction in the treatment of liver stagnation and spleen deficiency syndrome of chronic hepatitis B may be related to the regulation of the immune function of patients.
