硝黄煎剂熏洗对混合痔术后并发症的干预作用

目的:观察硝黄煎剂熏洗对混合痔术后并发症的干预作用,为临床治疗混合痔术后并发症提供参考。方法:将240例混合痔术后并发症患者按照随机数字表法分为治疗组和对照组各120例,治疗组从术后第2天开始采用硝黄煎剂熏洗,对照组同时间开始采用痔疾洗液熏洗,两组均每天熏洗2次,共熏洗12 d。比较两组患者术后第1、3、7、12天及创面愈合时肛缘水肿评分、肛门坠胀疼痛程度、便血评分、创面渗出评分,治疗前及治疗后1个月生活质量评分,以及住院时间、住院费用,并于治疗结束时评价两组临床疗效。结果:两组肛缘水肿评分、肛门坠胀疼痛程度、便血症状评分、创面渗出评分均随手术时间的延长有显著改善(P<0.001)。术后第3、7、12天及创面愈合时,治疗组肛缘水肿评分、肛门坠胀疼痛程度改善优于对照组(P<0.05);术后第3、7、12天,治疗组便血评分、创面渗出评分优于对照组(P<0.05);术后1个月,两组患者生活质量评分较治疗前均明显提高(P<0.001),且治疗组患者生活质量评分高于对照组(P<0.001);治疗组住院时间短于对照组(P<0.001),住院费用低于对照组(P<0.001);治疗结束时,治疗组疗效高于对照组(P<0.001)。结论:硝黄煎剂熏洗治疗混合痔术后并发症在缓解肛缘水肿、肛门坠胀疼痛、便血、创面渗出及改善患者生活质量方面优于痔疾洗液,且硝黄煎剂熏洗治疗可缩短住院时间,费用低廉。

Intervention Effect of Fumigating and Washing with Xiaohuang Decoction（硝黄煎剂） on Postoperative Complications of Mixed Hemorrhoids

Objective:To observe the intervention effect of fumigating and washing with Xiaohuang Decoction on postoperative complications of mixed hemorrhoids,so as to provide reference for clinical treatment of postoperative complications of mixed hemorrhoids.Methods:A total of 240 patients with postoperative complications of mixed hemorrhoids were divided into treatment group and control group according to random number table method,with 120 cases in each group. The treatment group was given fumigating and washing with Xiaohuang Decoction(硝黄煎剂 )from the 2 nd day after the operation,while the control group was given fumigating and washing with Hemorrhoid Lotion(痔疾洗液) at the same time. Both groups were treated twice a day for 12 days in total.Comparisons of anal edema score,degree of anal bulge and pain,bloody stool score,wound exudation score were made between the two groups of patients on the 1 st,3 rd,7 th and 12 th days after surgery. The life quality scores before and 1 month after treatment,as well as duration of hospitalization and cost of hospitalization of the two groups were also compared. The clinical efficacy of the two groups was evaluated at the end of the treatment. Results:All the indicators of the two groups,including anal edema score,degree of anal bulge and pain,bloody stool score,and wound exudation score had a better trend with the extension of the operation time(P<0.001). On the 3 rd,7 th and 12 th day after the surgery,and at the time of wound healing,the improvements of anal edema score,and degree of anal bulge and pain in the treatment group were better than those in the control group(P<0.05). On the 3 rd,7 th and 12 th day after the surgery,the bloody stool score and wound exudation score of the treatment group were better than those of the control group(P<0.05). One month after the surgery,the life quality score of the two groups were significantly higher than those before the surgery(P<0.001),and the life quality scores of the treatment group were better than those of the control group(P<0.001). The duration of hospitalization of the treatment group was shorter than that of the control group(P<0.001),and the cost of hospitalization of the treatment group was lower than that of the control group(P<0.001). At the end of treatment,the total effective rate of the treatment group was higher than that of the control group(P<0.001). Conclusions:In the treatment of postoperative complications of mixed hemorrhoids,fumigating and washing with Xiaohuang Decoction is superior to Hemorrhoid Lotion in alleviating anal edema, anal bulge and pain, bloody stool, wound exudation and improving the life quality of the patients. Moreover,fumigating and washing with Xiaohuang Decoction can shorten the duration of hospitalization and reduce the cost of hospitalization.
