总结卢尚岭教授对慢性心衰病机认识与治疗经验。心衰是各种心病日久的常见转归,其危害严重。由于导致心衰的疾病不同,因而患者发病年龄、体质、病程与病性有明显差异。心气心阳虚损,乃至衰竭,推动无力,气化失司,以致血瘀水停,是心衰发病的共同病机。五脏俱病,病位在心;心脏受邪,体用俱损。正虚邪实互为影响,心气心阳不足,正虚不充,必然导致气滞、痰浊、瘀血、寒凝、浊滞、水停等邪气内生,邪甚伤正。因此,治疗心衰强调补虚扶正与祛邪泄浊兼顾,以益气温阳、活血通脉、行水泄浊为治疗原则。创制心衰1号方、心衰2号方、益心汤等,并随证加减,常获满意疗效。

Experience by Professor LU Shangling in Treatment of Chronic Heart Failure

The experience by Professor LU Shangling in the disease mechanism and treatment of chronic heart failure was summarized. Heart failure is a common outcome of all the heart diseases after a long time,and is very severe. Since various diseases could lead to heart failure,significant differences were present in the age,physique,course and nature of disease. Due to deficiency of heart qi and/or heart yang,even collapse of heart qi and/or heart yang,the heart fails to promote blood circulation,and disorder of qi transformation,and finally there would be blood stasis and water stagnation,which is the common disease mechanism of heart failure. Heart failure involves five-zang organs and the disease location is at the heart. When the heart is affected,both the organ and function are damaged. Healthy qi deficiency and evil qi excess affect each other,which will lead to deficiency of heart qi and heart yang. Deficiency healthy qi leads to qi stagnation,turbid phlegm,blood stasis,cold coagulation,turbid stagnation and water stagnation,which further damage the healthy qi. Therefore,in the treatment of heart failure,both tonifying deficiency and replenishing healthy qi and dispelling pathogen and removing turbid should be applied,to replenish qi and warm yang,activate blood circulation and dredge vessels,move water flow and remove turbidity. Professor LU invents Xinshuai Yihao Formula(心衰1号方),Xinshuai Erhao Formula(心衰2号方),Yixin Decoction(益心汤),which are modified based on the syndrome differentiation in clinic to get satisfactory results.
